A Brief Overview

Responsible for all clinical services supporting the inpatient and outpatient oncology and and infusion therapy practice settings.

What you will do

•  Establish and/or participate in a clinical practice by interacting with patients, prescribers, nurses, and other healthcare professionals, as a member of an interdisciplinary team on patient care units to provide pharmacotherapeutics recommendations and monitoring for optimal medication use

•  Identify and develop medication use evaluations to develop or update policies to ensure appropriate medication use and regulatory compliance

•  Functions independently in day to day clinical and administrative duties associated with the position, consulting with pharmacy management only when required. In general, uses a high degree of professional judgment to solve complex problems which routinely arise throughout the work day

•  Implements risk mitigation strategies and review of laboratory and other tests to help determine efficacy and toxicity of medication therapy

• Design, implement, assess, monitor and document therapeutic plans utilizing the most effective, least toxic and most economical medication treatments

•  Provide pharmaceutical care to patients in direct care setting to achieve therapeutic goal and make referrals as needed to optimize patient outcomes and treatment success

•  Identify and collaborate on departmental and hospital wide continuous quality and performance improvement projects and initiatives to ensure financial stewardship

•  Lead and participate in education of pharmacy staff, pharmacy students, nursing, and other healthcare professionals

•  Lead and participate in hospital wide committees and clinical research related to area of practice

•  Serve as a preceptor for pharmacy students and PGY-1 Residents

•  Performs all other duties as assigned

Education Qualifications

  • Doctorate Doctor of Pharmacy Degree with completion of an accredited PGY-2 Residency Certificate or equivalent clinical pharmacy and leadership experience required

Experience Qualifications

  • Minimum 2 years hospital pharmacy with clinical oncology experience

Knowledge, Skills, and Abilities

  • Strong communication, project management
  • Pharmacy residency with strong oncology component

Licenses and Certifications

  • PHARMD - Licensed Pharmacist License to dispense Pharmaceuticals in the State of New Jersey upon hire required
