Status: Full Time 80 Hours

Shift:  Day/ Evening

SALARY RANGE: 132,953.60 - 221,457.60YEARLY

General Summary:

The Emergency Department Clinical Pharmacy is responsible for the provision of comprehensive clinical pharmacy services for the emergency department. This includes pharmacokinetic and therapeutic consultations, code and trauma participation, and nursing education for new therapies.

Responsibilities:

1. Conducts staff development programs or educational services to pharmacy staff, pharmacy students and residents, nursing staff, patient care services, physicians, medical residents, and other healthcare professionals
2. Serves as a subject matter expert in emergency medicine, completing drug information questions for the emergency department.
3. Provide comprehensive pharmacy review of emergency department medication orders including medical emergencies, code resuscitation efforts, and high-risk procedures. Comprehensive medication reviews includes but are not limited to order verification, appropriate indication, dosing, allergy assessment, organ function, IV to PO, antimicrobial de-escalation and drug interactions
4. Provides patient specific medication education using teach back method for discharge medications when appropriate
5. Serve as a well-integrated member of the ED multidisciplinary team actively participating in patient care decisions including bedside management of medical emergencies (e.g., trauma, stroke, psychiatric, toxicological), resuscitations, transition of care, pain management, assisting with opioid use disorder, and discharge medication reconciliation to optimize pharmacotherapy for ED patients.
6. Identify and evaluate medication-related problems based on clinical presentation, available history, or laboratory data.
7. Review patient antimicrobial cultures and perform patient antibiotic call-backs for patients who were discharged untreated or on inappropriate empiric antibiotics. Call patients diagnosed with sexually transmitted diseases that have been identified by the Delaware Public Health Department as needing follow-up.
8. Review Emergency Department medication error reports to identify trends, system failures and gaps in medication use process and perform quality assurance activities to promote safe and effective medication use.
9. Serves as a preceptor to pharmacy residents and students
10. Actively participate in Emergency Department meeting, education, training.
11. Promote and demonstrate rational, cost-effective drug therapy. Focus on cost avoidance and cost savings due to medication use in the Emergency Department.
12. Establish relationships with local, state, and national organizations/societies to stay informed about community resources and participate in relevant initiatives.
13. All other duties as assigned within the scope and range of job responsibilities.
 

Required Education, Credential(s) and Experience:

  • Education: Bachelor Degree Pharmacy ; In additional to BS in Pharmacy, position also requires, any BPS Pharmacy Board Certification, and certificate in Emergency Medicine or Critical Care.
  • Credential(s): Licensed Pharmacist; Basic Life Support ;Advanced Cardiovascular Life Support ;Pediatric Advanced Life Support; Must be eligible for Licensure in the State of Delaware as a Pharmacist.
  • Experience: Required:Two years of hospital experience and completion of clinical training.

     

Preferred Education, Credential(s) and Experience:

  • Education:  Doctorate Degree Pharmacy
    Preferred: PharmD with Post Graduate Year 2 Residency in Critical or Emergency Medicine ~or~ board Certification in Critical Care Pharmacy or Emergency Medicine Pharmacy.

  • Experience: Preferred: Completion of PGY-2 ASHP-accredited Pharmacy Residency in in Critical or Emergency Medicine. ~OR~Completion of PGY-1 ASHP-accredited Pharmacy Residency and a minimum of 3 years of experience in a related area of expertise AND BPS board certification in Critical Care Pharmacy or Emergency Medicine Pharmacy. ~OR~Minimum of 5 years of experience in a in Critical or Emergency Medicine AND BPS board certification Critical Care Pharmacy or Emergency Medicine Pharmacy.
