The Emergency Department (ED) Clinical Pharmacist Specialist provides comprehensive pharmaceutical care services to ED patients with the primary goal to improve patient outcomes.  Collaboration with physicians, residents and other healthcare providers, combined with direct patient care activities, will allow the ED pharmacist an opportunity to promote and improve safe medication use in the ED that is evidence based and aligned with national quality indicators.

Education:

  • Graduate of an ACPE-accredited School of Pharmacy required
  • Doctor of Pharmacy degree (Pharm.D.) required
    • Bachelor’s degree in Pharmacy with 5 years hospital based, patient care experience in ED, Trauma or Critical Care, considered in lieu of Pharm.D.
  • Advanced Pharmacy Residency Training (PGY1) highly preferred
  • Advanced Pharmacy Residency Training (PGY2) in critical care or emergency medicine preferred

Licensure/Certification/Registry:

Licensed pharmacist in the State of Illinois required

  • Board Certification in Pharmacotherapy (BCPS) required
    • PGY-1 residency accepted in lieu of BCPS requirement
    • BCPS certification required within 12 months of hire
  • Board Certification in Critical Care (BCCCP) or Emergency Medicine (BCEMP) preferred
    • BCCCP or BCEMP certification negates requirement for BCPS.
  • Basic Life Support Certification (BLS) required
  • Advanced Cardiac Life Support Certification (ACLS) required
  • Pediatric Advanced Life Support Certification (PALS) required within 6 months of hire
  • Emergency Neurological Life Support Certification (ENLS) required within 12 months of hire

Experience:

  • Minimum of three years relevant hospital based, patient care experience required
    • Completion of a PGY-1/PGY2 pharmacy residency program will satisfy this requirement

Responsibilities

 

  1. Work collaboratively with ED physicians, residents and other healthcare providers to promote and improve safe medication use. Consult with physicians and other health care providers to develop and implement a medication plan that meets the overall goals of patient care established by the health care team. Assist with drug therapy management in medical emergencies (i.e. codes, trauma, stroke). Assist the nurse in gathering accurate home medication lists in complicated, admitted patients. Represents pharmacy at various interdepartmental meetings (i.e. Unit Based Council, ED Committee, etc.). Serve as a liaison between the pharmacy and ED to ensure the timely administration of medications
  2. Participate in the development, implementation and monitoring of evidence based, medication use guidelines that meet National criteria for quality measures such as in Heart Failure, Stroke, AMI, Pneumonia and Sepsis
  3. Provide patient specific medication use teaching in high risk patients (i.e. Poly-pharmacy, pediatric patients, patients on high risk medications or who have been identified as high risk for readmission)
  4. Ensures appropriateness, effectiveness, and safety of each patient’s medication regimen in order to manage all aspects of medication use for patients in the ED
  5. Documents all recommendations and interventions in the electronic medical chart
  6. Prevent, detect, monitor, and report adverse events (medication errors and adverse reactions).
  7. Serves as an educational resource for patients, pharmacy colleagues, students, residents, physicians, nurses and other healthcare professionals.
  8. Performs order entry, order verification or clinical order review
  9. Serves as a pharmacy representative to various Hospital committees or project work groups as they relate to clinical pharmacy-ED practice. Provides Pharmacy & Therapeutic Committee support through presentations for formulary management or quality improvement as needed
  10. Actively participates in the professional development of pharmacy students and pharmacy residents
  11. Meets and maintains the requirements to serve as a pharmacy residency preceptor
  12. Ensures compliance with all local, state and national regulatory requirements and practices within Springfield Memorial Hospital’s policies and procedures.
  13. Participate in or lead at least 1 quality improvement, medication safety or emergency preparedness initiative on an annual basis
  14. Maintains professional growth and development through continuing education, seminars, workshops and professional affiliations.
  15. Provide formal medication related consultations (i.e. Pharmacokinetic consultations) as required
  16. Performs other related work as required or requested.
