Anticipated responsibilities include but are not limited to
Participate in direct patient care, providing comprehensive acute management of patients.
- Provide clinical support and interdisciplinary education to multidisciplinary healthcare teams.
- Review medication profiles to assure optimal pharmacotherapy treatment plans and optimize medication therapies to prevent and reduce medication errors.
- Provide discharge counseling to patients and caregivers as needed.
- Contribute to the development of medication guidelines, treatment plans, protocols, and optimization of Electronic Medical Record.
- Collaborate with oncology clinical manager team to support oncology-related quality improvement and clinical workgroup initiatives.
- Support the pharmacy department's clinical research initiatives and serve as a preceptor and mentor for PGY1 pharmacy residents, PGY2 pharmacy residents, and pharmacy students from local Colleges of Pharmacy.
Qualifications
- Doctor of Pharmacy (Pharm.D.) degree from an ACPE accredited School of Pharmacy.
- Active pharmacist license in New York, or eligible for licensure.
- Completion of a PGY2 residency in Oncology.
- Board certification in Oncology Pharmacy (BCOP) preferred.
- Excellent communication, interpersonal, and teamwork skills.
