ChristianaCare is recruiting for a Care Standardization Clinical Pharmacy Specialist to work part time, 40 hours bi-weekly. In this position, you will be responsible for the review, development, implementation and coordination of medication related order sets, care guidelines and protocols for Pharmacy Services.

Principal Duties and Responsibilities:
  • Serve as subject matter expert for Pharmacy services and acute care treatment guidelines and recommendations related to inpatient medication management
  • Coordinate the review and updates of inpatient care guidelines, order sets, and policies / procedures with specific emphasis on Pharmacy and medication management
  • Collaborate with relevant stake holders during review process to ensure appropriate multi-disciplinary feedback is acquired as appropriate
  • Collaborate with pharmacy stakeholders especially regarding operational needs, supply chain management, informatics, formulary management, etc. to ensure departmental cohesiveness is obtained
  • Active participant in pre and post changes related to medication management as it relates to medication procurement, prescriber ordering, order verification, administration, and documentation
  • Assist in identifying trends in medication use and communicate trends to Pharmacy leadership
  • Advise procurement supervisors on procedural changes, utilization and development of staff, implementation of productivity goals and make recommendations for improvements in inventory based on expected trends in medication use
  • Provides routine feedback with respect to program impact and areas for improvement


Education and Experience Requirements:
  • PharmD from an ACPE-accredited School of Pharmacy required
  • ASHP-accredited PGY1 Pharmacy Practice Residency required or equivalent experience (3 years of acute clinical pharmacy experience)
  • ASHP-accredited PGY2 Pharmacy Residency (relevant acute care specialty) required or equivalent experience (3 years of acute clinical pharmacy experience, in addition to PGY1 residency or equivalent)
  • Experience in order set review and development preferred
  • Board Certification (example, BCPS) required within 2 years of hire
  • Current licensure (or eligible to reciprocate) in the State of Delaware
