You will utilize your clinical knowledge and a strong clinical team in a progressive healthcare environment to lead the clinical pharmacy programs. Responsibilities include but are not limited to:
- Leading clinical pharmacy programs and services including but not limited to Pharmacy & Therapeutics Committee, Pain stewardship, Hospital Medicine Team meetings
- Developing drug policy to support optimal pharmaceutical care by assessing, developing, coordinate, implementing, and maintaining clinical pharmacy programs and services.
- Developing and implementing an Annual Clinical Plan with the Director of Pharmacy and the Regional Vice President of Clinical Service.
- Provide leadership in the development and maintenance of drug policy that promotes optimal cost-effective pharmaceutical care for all patients.
- Contribute to development and maintenance of clinically relevant tools to support clinical programs and aid the Director in maintaining the pharmacy.
This position is scheduled to work 40 hours a week primarily daytime hours with occasional evenings and weekends based upon business needs. The pharmacy is open 24/7.
Memorial Health System is a not-for-profit integrated health system that includes a 200-bed acute care hospital, 25 bed critical access hospital, outpatient infusion center, and two stand-alone ER’s. Qualifications:
- Bachelor of Science in Pharmacy OR Doctor of Pharmacy (Pharm.D.) from an ACPE accredited school of pharmacy
- One year of clinical hospital pharmacist experience
- Ohio pharmacist license in good standing (or Ohio Licensure eligible within 90 days)
Preferred Qualifications:
- Residency OR Board Certification OR;
- 3+ years of clinical hospital experience.
