Henry Ford Health is seeking qualified candidates to expand Ambulatory Clinical Pharmacy Services. The Ambulatory Clinical Pharmacy Specialist responsibilities include, but are not limited to:
- Provides comprehensive medication and disease state management activities via a Collaborative Practice Agreement (CPA), as an integral part of the care team. The expanded scope of practice includes initiating, modifying, discontinuing medications, ordering vaccines, ordering and evaluating laboratory data, and designing and implementing medication, monitoring, and adherence plans. Collects and reviews patient medical and medication histories, assess vital signs, facilitate ordering of medical equipment, place referral for patient follow-up in specialty clinics/departments, and facilitate prior authorization with appropriate pharmacy partners across the system. Provides disease state and medication education and provide care coordination and transition of care services. Using a data-driven approach to decision making, will support population health strategies and participate in value-based initiatives. Will serve as a conduit between HFH acute care, outpatient, mail order and specialty pharmacies and the patient’s provider. To decrease total cost of care, improve patient safety and satisfaction, and clinical outcomes, will appropriately insource prescriptions and specialty medications internally. Will participate in ongoing research, scholarship, quality initiatives, projects, committees, policy development, and formulary management strategies. Provides exemplary services through the provision of pharmaceutical care to individual patients and/or their population.
Qualifications include:
- Registered pharmacist in the State of Michigan. Doctor of Pharmacy degree from an ACPE accredited College of Pharmacy. Required: PGY1 Pharmacy Practice Residency and PGY2 Ambulatory Care Residency and history of providing top-of-license clinical services under a collaborative practice agreement (equivalent clinical experience (preferably 3-5 years) in ambulatory care setting may be considered in lieu of PGY2 Residency). Required: Board Certified Ambulatory Care Pharmacist (BCACP) (Board Certified Pharmacotherapy Specialist (BCPS) considered) or eligible to take BCACP exam and pass within 1 year of hire.
- Those practicing in primary care should have experience in various disease states including, but not limited to diabetes, hypertension, dyslipidemia, asthma, COPD, and congestive heart failure. Those practicing in nephrology/dialysis should have experience in this practice area. Prior experience and knowledge of integrated health systems.
