Team Impact

In this role, you will join an experienced team of healthcare professionals with over 20 years of emergency pharmacy experience. You will have the opportunity to form impactful relationships with a diverse group of healthcare professionals including various medical and nursing staff outside of pharmacy. 

In this role, you will work among an interdisciplinary team to improve patient outcomes through direct bedside care and provide leadership for medication reconciliation in the Emergency Department.  You will be responsible for coordinating various clinical programs/initiatives to optimize drug use, improve therapeutic outcomes, prevent drug related problems/medication errors, and reduce associated costs. In summary, your work will have a substantial impact on the development and implementation of mechanisms that promote safety and quality related to medication management at Glens Falls Hospital. Some position highlights include:

  • Full time 7-on-7-off with 3:00 PM– 1:00 AM schedule (Great for accruing shift differentials!)

Responsibilities

  • Provide direct patient care to all adult or pediatric patients as needed in the emergency department
  • Respond to all emergency codes to assist with pharmacological management including optimal drug selection and preparation, dosing recommendations, and monitoring for effectiveness and adverse reactions
  • Provide direct clinical review and evaluation of patient specific therapy regimens in the acute care setting and coordinate necessary drug therapy adjustments for individual patients
  • Provide direct patient care contact for visual assessment of physical and psychological signs of drug-related effects
  • Provides drug and clinical information to other healthcare professionals in response to new and evolving issues related to the care of patients in the emergency department
  • Routinely providing medical, pharmacy and nursing staff education/training to promote clinical competency and to ensure appropriate, safe, and cost-effective medication therapy

Qualifications/Experience

  • B.S. in Pharmacy required
  • Pharm. D. highly desired
  • PGY2 in Emergency Medicine/ Critical Care, or 3 years equivalent experience
  • Current, active license to practice pharmacy in New York State; or eligible for reciprocate licensure from another state required
  • ACLS and PALS certifications required
  • BCPS, BCCCP, or BCEMP preferred

Skills/Abilities:

  • Knowledge of current pharmacy principles and practices related to medication reconciliation, and drug therapy review
  • Knowledge of pharmacy principles in critical care, toxicology and emergency care setting
  • Knowledge of regulatory requirements and standards
  • Ability to work in a fast-paced environment, respond to questions quickly and accurately and adapt to rapidly changing situations
  • Ability to work independently within established policies and procedures
  • Ability to accomplish established program goals and objectives
  • Ability to exercise judgment, tact, and diplomacy
