Overview

The Dartmouth-Hitchcock Pharmacy in Lebanon, NH seeks a full-time Clinical Pharmacist to join our Medical Oncology team. The Clinical Pharmacist Lead for Medical Oncology is responsible for the oversight of clinical pharmacy services and direct patient care activities in the solid tumor oncology space.

This is a full time 40/hr week benefited position. Day shift.

Responsibilities

* Maintains an active clinical practice by establishing a patient clinic in a solid tumor oncology disease state
* Completes monograph and drug class review development for solid tumor oncology and supportive care agents
* Presents updates on new solid tumor oncology drug approvals to clinical pharmacists and nursing staff
* Assists the Clinical Pharmacy Manager in the development, implementation, and maintenance of clinical pharmacy services for solid tumor oncology patients (e.g., creation of new pharmacy collaborative practice agreements, expansion of patient education services)
* Conducts medication use evaluations for solid tumor oncology agents
* Develops new and maintains existing Beacon order sets for solid tumor disease states
* Serves as the primary clinical pharmacy resource for solid tumor oncology
* Serves as the primary trainer for new pharmacist hires in the ambulatory oncology space
* Serves as a Tri-Chair for the Hematology/Oncology Drug & Practice Specialty Panel
* Serves as the PGY2 Ambulatory Oncology I rotation preceptor
* Performs other duties as required or assigned.

Qualifications

* Graduate of an ACPE - accredited School of Pharmacy.
* 1 year of combined clinical pharmacy/specialty and/or residency experience required.
* Pharm D preferred.
* Must have demonstrated ability to work independently with minimal direct supervision, with the ability to set priorities and make critical decisions.
* Effective oral and written communication skills, as well as demonstrated interpersonal and team skills essential.

Required Licensure/Certifications

* Must have a current, unrestricted pharmacist licensed to practice in the state of NH
* Must obtain Board Certification in Oncology Pharmacy (BCOP) within 3 years of hire and VT Telepharmacist licensure within 3 months of hire

* BLS Certification within 30 days of hire for Clinical Pharmacist Leads in the Retail & Specialty Pharmacy if required to administer immunizations
