POSITION SUMMARY

Hospital Pharmacists:

The Pharmacist ensures accurate and timely distribution of medications and other pharmaceutical supplies according to established procedures and standards, in support of high quality patient care. The Pharmacist interprets, evaluates and verifies medication orders accurately assessing for drug-drug, drug-allergy and drug-disease state interactions. The Pharmacist provides drug information to clinicians and patients. It is the responsibility of the Pharmacist to maintain accurate and complete records on all patient profiles and controlled drug substances as required by law. The Pharmacist properly manages pharmaceutical inventories and maintains emergency drug supplies, automated dispensing machines and general medication security on the nursing units. This position directs and verifies the Pharmacy Technician's work. Pharmacists will be responsible for maintaining competency and participating in various clinical programs such as Anticoagulation Management, Pharmacokinetic Monitoring, Adverse Drug Event Monitoring, patient care rounding, pharmacotherapy optimization and general medication safety. This position supports organizational goals and values by providing quality customer service, participating in performance improvement efforts and demonstrating a commitment to teamwork, respect and cooperation. The Pharmacist actively pursues opportunities for professional growth and development.

Outpatient Pharmacists:

The Pharmacist ensures accurate and timely dispensing of medications and other pharmaceutical supplies according to established procedures and standards, in support of high quality patient care. The Pharmacist interprets, evaluates and verifies medications and dispenses drugs and pharmaceutical supplies to fill written and e-prescriptions. The Pharmacist provides drug information to clinicians and patients. It is the responsibility of the Pharmacist to maintain accurate and complete records on all prescriptions filled and controlled drug substances as required by law. The Pharmacist properly manages pharmaceutical inventories, maintains adequate pharmaceutical supplies, and provides patient counseling and medication therapy monitoring. This position directs and verifies the Pharmacy Technician's work. This position supports organizational goals and values by providing quality customer service, participating in performance improvement efforts and demonstrating a commitment to teamwork, respect and cooperation. The Pharmacist actively pursues opportunities for professional growth and development.

QUALIFICATIONS

EDUCATION: Doctor of Pharmacy Degree or Master's degree required.

LICENSE/CERTIFICATION: CurrentNew JerseyState license as a Registered Pharmacist required. ACLS Preferred, CPR Preferred for Hospital Pharmacist.

EXPERIENCE: Two years experience preferred. Two years hospital experience preferred for Hospital Pharmacist. Demonstrates an in-depth understanding of work processes. Proficiency in Clinical Applications preferred at time of hire; incumbents within position will be trained appropriately and then skill will be assessed and evaluated for this position within 90 days from date of hire.

PERFORMANCE EXPECTATIONS

Demonstrates the ability to perform the department specific competencies as listed on the Assessment and Evaluation Tool.

WORK ENVIRONMENT

Potential for exposure to the hazards and risk of the hospital environment, including exposure to infectious disease, hazardous substances, and potential injury.

Requires the ability to stand approximately 60% of the time and requires occasional reaching.

REPORTING RELATIONSHIP

Hospital Pharmacists report to the Director of Pharmacy Operations and provides direction to the Pharmacy Technician. Outpatient Pharmacists report to the Director of Outpatient Pharmacy and Business Development and provides direction to the Pharmacy Technician.
