Advanced Practice Pharmacists

Sutter Health’s Pharmacotherapy Clinic (PTC) demonstrates how the role of the pharmacist has evolved and expanded over the years to become a valued member of the integrated care team. Today’s Advanced Practice Pharmacist (APP) works in close collaboration with physicians and other clinicians to provide exceptional high-quality, personal care to our patients and improve clinical outcomes. For example, an oncology APP brings expertise in medication management and optimization and serves as a liaison between the patient and the oncology physician in an outpatient setting. Under a collaborative practice agreement, pharmacotherapy clinics leverage pharmacist resources to support both the patient—and physician—throughout the disease management journey. APPs also provide guidance on other medications to manage pain, adverse reactions and drug-drug interactions. Patients may also have comorbidities—such as diabetes or hypertension—requiring medication monitoring and adjustments as needed.
