Ambulatory Care Clinical Pharmacy Specialist

The Clinical Pharmacy Specialist works under minimal supervision. A clinical pharmacy specialist provides expertise to the department not routinely available from the pharmacy staff. The abilities of this individual may include a broad background in pharmaceutical care, specifically in clinical pharmacy practice, with knowledge applicable to both inpatient and ambulatory care settings.

The clinical pharmacy specialist provides pharmaceutical care services and clinical expertise using approved disease state management protocols in collaboration with other health professionals, managing patient drug therapy/pharmacokinetics/nutrition by consult, providing patient education, and promoting high quality, evidence-based, cost-effective medication therapy strategies through clinical recommendations. The clinical pharmacy specialist participates in multi-disciplinary settings and team rounds to optimize patient care while educating providers, residents, and students. Performs these duties in the clinical areas as assigned.
