Job Description: AMBULATORY PEDIATRIC COMPLEX CARE CLINICAL SPECIALIST:  The Division of Pharmacy of Cincinnati Children’s Hospital Medical Center (CCHMC) is seeking a clinical specialist to be part of a 27 person clinical specialist team.  This position provides coverage for the patients in our Complex Care Clinic. It involves coordination among various specialties and involvement with transitions of care. Patients seen in this clinic may have technology dependence, neurologic disorders, chronic respiratory diseases, and other chronic illnesses.  Responsibilities include identifying any drug related problems such as drug interactions, inappropriate doses, compliance issues, lack of indication for prescribed therapies, duplicate therapies, ensuring appropriate monitoring of specific therapies for efficacy and toxicity and recommending therapy modifications based on monitoring results. Responsibilities also include performing several medication management telehealth visits per week in conjunction with a provider. Coverage of patients seen during evening clinic hours is based on assessment of the pharmaceutical care needs of the patients scheduled.

This position will develop recommendations for evidence-based guidelines for specific pediatric populations followed in the clinic. Other responsibilities include medical and pharmacy staff education, drug use evaluation and performance improvement.  Preceptorship of pharmacy residents and pharmacy students is required.  Involvement in scholarly activity is expected. 

This position will provide inhouse weekend coverage of general pediatrics patients approximately one weekend per quarter.

Applicants should have earned a Pharm.D. degree and completed two years of residency with either one year in pediatrics and ambulatory experience, or with one year of ambulatory care with rotations in pediatrics.  Must be eligible for licensure in Ohio.
