Clinical Pharmacist- Hospital

Plays a vital role in patient care treatment through drug therapy services provided at Client.
These services are age-appropriate, for neonates, pediatric patients, adolescents, adults, and geriatric patients, performed when needed through direct contact with the patient, and/or in-direct association with or in support of health care providers.
Integrates the provision of pharmaceutical care with other elements of the patient care process to provide a collaborative, cooperative effort with other members of the health care team to assure optimal drug therapy outcomes.
Performs all duties in a manner which aligns with the department and organizational mission, vision, and values of Client.

Requirements

Minimum Years of Experience: 2.
