Applications are invited from qualified individuals with training and experience in ambulatory care or patient-centered medical home (PCMH) pharmacy practice for a fulltime, non-tenure-track, 12 month appointment at the rank of clinical assistant or associate professor in the School of Pharmacy with a secondary appointment in the School of Medicine. Faculty rank is commensurate with academic background and practice experience.

Responsibilities: The principal responsibility is to establish and maintain a direct patient care practice in a WVU Medicine PCMH in Morgantown, WV. As the pharmacist member of the interprofessional PCMH team, the faculty member will be responsible for comprehensive medication management for individual patients, especially for chronic diseases (e.g., diabetes group clinic, COPD clinic, asthma, anticoagulation, dyslipidemia, obesity, osteoporosis, hypertension, tobacco use disorder). The individual will also participate in population health and wellness activities such as developing medication use policies and procedures, creating and expanding chronic care management programs, and participating in quality improvement activities. The individual also has teaching, research/scholarship, and service responsibilities within the School of Pharmacy. 

Credentials: Doctor of Pharmacy degree from an ACPE-accredited institution and PGY1 pharmacy residency training are required. Completion of a PGY2 pharmacy residency in a family medicine that includes the PCMH concept or a related area is preferred. Pharmacy licensure in West Virginia will be required. The successful candidate will possess a high level of clinical practice skills, enthusiasm for didactic and clinical teaching, and the interpersonal skills needed for interacting effectively with students, faculty, health professionals, and patients.

Institution and Location: The main practice site is the Clark K. Sleeth Family Medicine Center at University Town Centre, a state-of-the-art outpatient care facility in Morgantown, WV. West Virginia University is a land grant, Carnegie-designated Doctoral Research/Extensive institution, with approximately 23,000 undergraduate, 5,000 graduate, and 1,600 professional students. The Health Sciences Center includes the Schools of Pharmacy, Dentistry, Medicine, Nursing, and Public Health. The Morgantown area has approximately 80,000 residents and is rated as one of the best small towns in the U.S., with affordable housing, excellent schools, a picturesque countryside and many outdoor activities.
