Clinical Pharmacist, Advanced Heart Failure/Cardiology

Atrium Health Wake Forest Baptist Medical Center currently searching for motivated candidates for a Clinical Pharmacist to provide patient care for Advanced Heart Failure (AHF)/Cardiology services.  This position is an integral part of a large pharmacy team and progressive group of pharmacy leaders who provide comprehensive pharmacy services throughout the medical center. The Department of Pharmacy’s vision is to enhance its pharmacy operations, advance clinical pharmacy programs, and expand interdisciplinary patient centered care services and pharmacy residency programs.  As a part of this growth and refinement of practice, the Department continues to seek talented and motivated individuals to add to its team. 

Atrium Health Wake Forest Baptist Medical Center is an 885-bed tertiary care hospital and campus in Winston-Salem. This campus is home to the adult and pediatric Level I Trauma and Burn Center, the 144-bed Brenner Children’s Hospital and 167-bed Comprehensive Cancer Center. Through these resources the hospital provides acute care to both adult and pediatric patients throughout the western region of North Carolina and specialty care to patients internationally.  WFBH admits approximately 60,000 inpatients each year, its emergency rooms have approximately 187,000 visits each year, and has close to 2 million outpatient visits at all ambulatory and outpatient departments. AHWFB is affiliated with Wake Forest School of Medicine and Wake Forest innovation Quarter.

Over 350 administrators, pharmacists, and technicians provide comprehensive pharmacy services spanning the continuum of care across multiple patient populations. AHWFB has a long history of clinical excellence and innovation in pharmacy practice. Today the department provides decentralized clinical services to more than 37 inpatient services and a variety of ambulatory clinics. Pharmacists and technicians partner to ensure safe transitions of care during patient admission and discharge and assist patients in accessing pharmaceutical care. In the ambulatory setting the Department of Pharmacy has community and specialty outpatient services as well as home infusion. Graduate Pharmacy Education consists of ten ASHP accredited residency programs, an internship program, and learning experiences for three schools of pharmacy in North Carolina.

The AHF/Cardiology Pharmacist is responsible for:

• Proficiently providing direct patient-centered care for ambulatory AHF patients and critically ill AHF/Cardiology inpatients
• Participates in all aspects of the medication-use system while providing comprehensive and individualized pharmaceutical care as a member of the interdisciplinary team
• Active membership and participation in multidisciplinary hospital committee is required as well as leadership in departmental clinical education and research activities
• Work closely with Clinical Pharmacy Managers, the Director for Acute Care Clinical Services to develop goals for the department
• These positions are responsible for actively identifying service related issues which require evaluation and facilitating research projects, quality improvement initiatives, or healthcare provider education as needed to advance practice
• External presentations, publications, and professional speaking is strongly encouraged and supported   
• As faculty within the Graduate Pharmacy Education program, he/she will serve as a preceptor for the Pharmacy Residency Programs
• Provides experiential mentorship for Doctor of Pharmacy students

In short, the AHF/Cardiology Pharmacist will facilitate medication therapy and pharmacy services to this specialized patient population both in clinic and admitted patients and collaboratively support direct patient-centered care.

Position requirements include:

1.             Education: Doctor of Pharmacy degree from an ACPE accredited College of Pharmacy

2.             Clinical training and experience: Completion of PGY1 Pharmacy residency and PGY2 Cardiology, Critical Care, or Transplant residency is preferred (or 2-4 years of clinical experience in critical care/cardiology patient populations may be considered in lieu of formal training and exceptional aptitude or knowledge may be considered in lieu of experience).

3.             Licensure/certifications: Licensed or eligible for licensure to practice pharmacy in the state of North Carolina required, Board of Pharmacy Specialties (BPS) certification preferred

4.             Eligibility to apply for and receive your Certified Pharmacy Practitioner (CPP) certification
