Job Summary: Health science specialist who assures the safe and appropriate use of drugs in patients through the application of specialized knowledge. Functions in patient care and uses judgment in the collection and interpretation of pharmacologic data, patient-specific involvement, and direct interprofessional interactions. This role of Clinical Pharmacist is patient-oriented, disease-oriented, drug-oriented, and has, in practice, an interdisciplinary orientation.

Minimum Requirements: Graduate of an accredited Doctor of Pharmacy Program with current licensure as a pharmacist in state of practice. Preferably have completed an ASHP accredited residency program or other advanced training. Must have the ability to use the Pharmacy computer system at an acceptable level after having been instructed during the training period. The Clinical Pharmacist must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his/her assigned unit. The individual must demonstrate knowledge of the principles of growth and development over the life span, possess the ability to assess data reflective of the patient's status, interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the Pharmacy/I.V. Admixture Department. Must be willing to work a rotation shift and be willing to accept "call back."
