Position Description:
Inpatient surgery pharmacist position at Mayo Clinic Hospital in Rochester, MN. In this role, the pharmacist provides exceptional care to patients throughout their peri-operative journey; including pre-operative, intra-operative, and post-operative phases of medication management. Decentral clinical services consist of collaborative practice with multidisciplinary teams, identification of drug-related problems, provision and documentation of recommendations through progress notes regarding optimization of drug therapy, admission and discharge medication reconciliation, transition of care planning, participation on discharge rounds, intermittent surgical team rounds, and direct patient care.
Job Responsibilities include assessment of medication therapy regimens, medication therapy management, medication order verification/entry, medication distribution, medication therapy monitoring, and consultation, documentation of medication therapy plans, and medication use education and pharmaceutical care services. Precepting of Mayo Clinic pharmacy and other disciplines’ learners is expected.
Involvement in scholarly activities including quality initiatives and multidisciplinary research is encouraged and supported through extensive institutional and departmental resources. Opportunity for academic appointment at hiring and progression through appointment ranks through achievement.
Qualifications
Qualifications:
Graduate of an accredited College of Pharmacy. Doctor of Pharmacy (Pharm.D.) degree or equivalent experience. Equivalent experience includes at least two years of relevant pharmacy practice experience.
**Visa sponsorship is not available for this position; also, Mayo Clinic does not participate in the F-1 STEM OPT extension program**
Additional Qualifications:
Commitment to excellence, accuracy, attention to detail and teamwork required. Ability to communicate effectively, work collaboratively with others, organizes time well, practice with an agile mindset, solve problems, and work independently with minimal supervision required.
The following are preferred:
- Previous experience including PGY1 or PGY2 training and/or equivalent experience of 3 years in practice
- Evidence of ongoing scholarly pursuits including research/quality improvement, publications, or scientific presentations
- Relevant applicable pharmacy experience working in a progressive and collaborative practice environment
- Experience and/or interest in teaching and/or precepting
- Board Certification in Pharmacotherapy Specialty (BCPS)
License or certification:
License to practice Pharmacy in the State of Minnesota or eligibility for licensure.
