Summary
The antimicrobial stewardship pharmacist is responsible for oversight of a program of coordinated services in the provision of safe, effective, appropriate and cost-efficient antimicrobial therapy across the enterprise at Children’s Minnesota. The successful candidate for this role will possess the necessary skill and knowledge to provide direct patient care as well as provide complex, in-depth infectious diseases clinical services, teaching, leading, motivating, consultation and administrative abilities with clinical staff.
Specific Responsibilities
1. Develop, enforce and maintain policies and procedures that promote and enhance patient outcomes through safe, effective, cost-efficient and appropriate antimicrobial therapy.
2. Coordinate programs designed to minimize medication errors, adverse drug reactions and medication misuse with regard to antimicrobials through reporting, analysis and follow-up.
3. In partnership with the Antimicrobial Stewardship Committee, coordinate and maintain an active antimicrobial stewardship program, including:
a. Overseeing daily prospective audit of antimicrobials;
b. Assisting in development and maintenance of an antimicrobial formulary approved by the professional staff;
c. Working with the infectious disease service to evaluate appropriate use of restricted antimicrobials;
d. Developing guidelines, policies and other monitoring and intervention strategies for antimicrobial use and streamlining prophylactic, empiric and definitive antimicrobial therapy;
e. Compiling data and metrics on antimicrobial use;
f. Maintaining the system and unit-based antibiograms, where appropriate;
g. Reporting data and findings to the Antimicrobial Stewardship Committee and the Pharmacy and Therapeutics Committee; and
h. Coordinating, facilitating and promoting education, training, competency assessment and performance improvement of pharmacists, pharmacy residents and other healthcare professionals in the appropriate use of antimicrobials.
4. Precepts pharmacy students and residents on an Infectious Diseases rotation.
Qualifications:
-Pharmacist licensure in the State of Minnesota current or within 3 months of hire, required
-Basic Life Support (BLS) certification current or within 3 months of hire, required
-Board Certification in Infectious Diseases Pharmacy, preferred
Education:
-Graduate from a college/school of Pharmacy accredited by the Accreditation Council for Pharmacy Education (ACPE) OR graduate from a foreign college of pharmacy that has at least a five-year curriculum and has passed the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and become certified by the Foreign Pharmacy Graduate Equivalency Commission (FPGEC), required
-Doctor of Pharmacy (PharmD) from an ACPE-accredited institution, preferred
-Completion of a residency or fellowship in Infectious Diseases or equivalent health-system experience, required
-Completion of a residency or fellowship in Pediatric Pharmacy, preferred
-Two-year minimum experience working in Antimicrobial Stewardship or Infectious Diseases, preferred
-Meets ASHP residency preceptor criteria, preferred
