UW MEDICAL CENTER - MONTLAKE PHARMACY has an outstanding opportunity for a FULL-TIME CLINICAL PHARMACIST - CARDIOLOGY (Rotating/Variable Shifts) to provide pharmaceutical care for our acute care cardiac patients.  Our Clinical Pharmacists are a vital part of the multidisciplinary patient care team. In addition to direct patient care responsibilities, our Pharmacists provide drug therapy consults, participate in treatment protocol development, provide patient/family/staff education, facilitate transitions of care, precept pharmacy students and residents, and perform duties of a Pharmacist 2.

RESPONSIBILITIES:

Clinical:
Drug therapy monitoring, including, but not limited to:
• Ensure appropriate drug therapy and nutrition, including indication, dose, drug interactions and drug allergies
• Evaluate pharmacotherapeutic regimens for effectiveness and adverse drug events, and intervene as needed
• Utilize the medical record and other appropriate documentation systems to communicate with healthcare professionals and to document specific pharmacotherapeutic recommendations
• Provide drug information and education to medical teams, nursing and dietician staff
• Provide patient/family counseling and education
• Update patient medication lists and reconcile medication orders upon admission and discharge

Education and Research:
• Provide primary preceptorship of pharmacy residents and student pharmacists
• Provide education to medical/surgical providers, students, and nursing staff
• Present at educational conferences to multidisciplinary staff, as requested
• Develop and provide nursing in-services
• Provide didactic teaching to student pharmacists at the University of Washington School of Pharmacy, as requested
• Participate in collaborative research projects with healthcare professionals, pharmacy residents and student pharmacists.
• Submit unique work for publication in professional journals

Practice Management:
• Implement and maintain departmental policies and procedures related to the safe and effective use of medications
• Routinely evaluate current updates in service specific literature
• Develop and maintain drug therapy monitoring systems for patients
• Establish and maintain efficient lines of communication between pharmacists, physicians, nurses and other members of the health care team.
• Participate in pharmacy and hospital committees as requested
• Participate in the development and implementation of quality assurance programs for pharmacy services, including, but not limited to, prevention of medication errors
• Perform drug utilization evaluation and cost containment analyses, as needed

Dispensing:
• Perform duties of a Pharmacist 2.

DESIRED:
• Experience in cardiology, and cardiac surgery and/or PGY2 residency is desirable.
