The Medication Management and Optimization Pharmacy Specialist is responsible for optimizing formulary management as well as providing a pharmacy clinical service to ensure safe, effective, appropriate, cost-effective medication therapies for inpatients. This position will be split between 50% decentralized clinical staffing and 50% medication management responsibilities. Responsibilities include coordination of the P&T system-wide committee and coordination of large volume pump optimization. In addition they will complete the following when needed: new formulary requests, REMS program monitoring, and medication use projects. There is also an education component with precepting ASHP accredited PGY1 residents on a combined medication management and safety rotation.
- BS Pharmacy; or Doctor of Pharmacy Degree (Pharm D)
- Post graduate year 1 [PGY1] in Pharmacy Practice
- Post graduate year 2 [PGY2] or equivalent clinical experience
- Day shift only, every 6th weekend
- Inpatient decentralized clinical shift 50% (5 day shifts in an inpatient area of choice per pay period)
