Overview:
The clinical pharmacy technician will enhance the quality and access of clinical pharmacy services by assisting in the delivery of medication management services to patients. The clinical pharmacy technician serves as an integral part of the multidisciplinary team and serves as the patient's advocate in addressing their pharmaceutical care needs. This position will serve as a fundamental team member to continue to cultivate CommUnityCare's team?based care model initiatives.

Responsibilities:
Essential Duties (at least 5 that are non-negotiable duties and are absolutely pertinent to successfully completing the job without accommodations):Primary Accountabilities:• Support clinical pharmacists in operating an ambulatory care clinical pharmacy service • Assist patients with medication access needs, which may include refill requests/rerouting prescriptions via Standing Delegation Orders (SDOs), clarifying medication instructions, instructing patients how to complete refills, contacting pharmacies to resolve formulary issues• Conduct and triage patient phone calls as related to clinical pharmacy services and referring to clinical pharmacy or medical provider if escalation of care is needed• Help patients navigate medication management services, close care gaps, attend scheduled appointments, and/or complete lab-work • Assist with patient intake for clinical pharmacy visits including scanning in supporting patient documentation such as patient self-monitoring logs into the electronic health record (EHR)• Verify & enter patient information in the EHR, such as, but not limited to demographics, insurance, and medications and review clinical pertinent information in the patient's health record • Complete point of care tests and vital sign checks under SDOs• Collect/Pull data reports for clinical review• Identify patients with uncontrolled diseases who are eligible for clinical pharmacy services from clinical reports• Empower patients and/or families to be an engaged and active participant in their care • Collaborate and communicate with all members of the care team in providing patient-centered care & to close medication access needs• Manage the completion of needed diabetic medical equipment and/or other formulary documentation forms for clinical pharmacist-initiated medications• Manage clinical pharmacists' schedules and consults • Restock clinic supplies• Educate/train Medication Refill specialists or Clinical Pharmacist technicians' trainees when needed• Performing other duties as assignedKnowledge/Skills/Abilities:Knowledge: • Healthcare office concepts, practices, policies, and procedures• Knowledge of state, local, and federal pharmacy laws • Medical/pharmaceutical concepts, terms and nomenclature• Understands disease state indications and lab results for medication therapy outlined in SDOs• Understands medication formulary coverage and prior authorization process• Microsoft Office Applications (Word, Excel, Outlook)• HIPAA and Joint Commission guidelines and regulations (preferred)• Bilingual in English/Spanish (preferred)Skills/Abilities: • Skilled in verbal and written communications, including telephone and email etiquette• Demonstrate ability to successfully navigate and document within EHRs• Critical thinking skills to independently resolve problems and knowing when to seek further clarification from supervisor, clinical pharmacists, or medical providers• Ability to work in a self-directed and organized manner under direct supervision of clinical pharmacists• Ability to prioritize tasks to complete work in a timely manner• Maintain strong attention to detail, accuracy, and timeliness • Maintain confidential patient files and documentation in the EHR• Ability to work with diverse groups of people• Develop and maintain favorable relationships and partnerships with co-workers• Interact respectfully and collaboratively with patients and their families, striving to develop favorable relationships with families • Ensure all actions, job performance, personal conduct, and communications always represent CommUnityCare in a professional manner • Complete all mandatory training and maintain appropriate credentials/licensure• Ensure all tasks provided and associated with patient care, patient administrative processes and related duties comply with all regulatory and accreditation standards including The Joint Commission, CommUnityCare Standard Operating Procedures and CommUnityCare Policies and Procedures• Uphold all company policies and procedures as well as the overall mission and values of the organization

Qualifications:
MINIMUM EDUCATION: High School Diploma MINIMUM EXPERIENCE: Six years of experience as a Pharmacy Technician 1 year of Microsoft Excel experienceREQUIRED CERTIFICATIONS/LICENSURE: Registered with the Texas State Board of Pharmacy National Certification with the Pharmacy Technician Certification Board (CPhT)REQUIRED COURSES/COMPLETIONS (e.g., CPR) CompetenciesCPRMedication Therapy Management Certificate Program within 6 months of hireMedication History Certificate Education/Training Program within 6 months of hireAnnual completion of 2 hours of required medication therapy related continuation education
