The University of Vermont Medical Center is seeking a Specialty Ambulatory Pharmacist to join their team in Burlington, VT.

This pharmacist role primarily resides in the outpatient transplant/nephrology clinic. Interdisciplinary teamwork includes participation in weekly transplant evaluations, provision of therapeutic drug monitoring, and medication review and education. Responsibilities also expand into the inpatient sector and include patient care rounds, drug monitoring, CMS notes and communication with the inpatient pharmacy team to ensure optimal coordination of care. Additionally, this pharmacist is involved with longitudinal maintenance of transplant-related order sets, monitoring standards, polices/guidelines and provides transplant education/training to new pharmacists and pharmacy residents.

Transplant/nephrology will be the primary focus of this job role, however, work within additional specialty pharmacy areas (e.g. endocrinology, women's health, neurology) may be requested pending ongoing workload. Actions taken must uphold standards set by specialty pharmacy accrediting bodies and CMS requirements.

 

JOB DESCRIPTION:

The OP Ambulatory Pharmacist Clinician is an advanced practice pharmacist responsible for assuring optimal drug use outcomes in high risk, high cost or complex patient populations. They apply specialized knowledge of pharmacotherapy to direct patient care, teaching, and consultation. Participates as a contributing member of ambulatory care teams such as rheumatology, neurology, infectious disease, oncology, nephrology, or primary care. Provides clinical, pharmacy, and medication expertise in each area including policy development, pharmacy operations, prior authorization support and medication therapy outcomes.

 

EDUCATION:

Doctor of Pharmacy and completion of an ASHP accredited residency program or equivalent experience preferred. Licensed pharmacist in the State of Vermont.

 

EXPERIENCE:

3 years of advanced practice experience in their area of expertise preferred or equivalent experience in the ambulatory care and/or inpatient pharmacy setting.

 

QUALIFICATIONS:

  • Pharmacist - State Issued
  • PRISM Training
