State of Franklin Healthcare Associates is looking for an ambulatory care clinical pharmacist to join their team in the growing and desirable community of Johnson City, Tennessee!

The Clinical Ambulatory Care Pharmacist serves as a medication expert for patients and advises physicians, nurses, and other health professionals on medication therapy decisions. They work directly with patients at their provider appointments to discuss medications in depth, address any concerns, and monitor the patients health and progress to optimize medication therapy. They also educate patients on the proper use of their prescriptions (including inhalers, or injected therapies) and over-the-counter medications.  Multiple chronic disease state interaction in collaborative appointments as well as involvement in quality metrics initiatives to improve star ratings on a clinical excellence team.


Essential Duties and Responsibilities

  • Assess patients, including identifying and prioritizing patient problems and medication-related needs.
  • Evaluate drug therapy for appropriateness, effectiveness, safety, adherence, and affordability.
  • Collaborate with members of the healthcare team to achieve optimal patient outcomes across the continuum of care.
  • Demonstrate and apply in-depth knowledge of pharmacology, pharmacotherapy, pathophysiology, and the clinical symptoms, and natural history of diseases and/or disorders.
  • Locate, evaluate, interpret, and assimilate scientific/clinical evidence and other relevant information from biomedical, clinical, epidemiological, and social-behavioral literature.
  • Use health care delivery systems and health informatics to optimize the care of individual patients and patient populations.
  • Resolve medication-related problems to improve patient/population health and quality metrics.
  • Proactively works with the Quality Improvement Team on various quality metrics to optimize population heath for SoFHA patients. 


Doctorate of Pharmacy degree from an accredited College of Pharmacy. Post graduate residency training (at least one year) or equivalent experience. PGY2 in ambulatory care or PGY1 focused in ambulatory care preferred.

Certificates, Licenses, Registrations

Holds and maintains valid Tennessee pharmacist license. Specialty Board Certified in Ambulatory Care Pharmacy (BCACP) encouraged.
