Job Description
General Summary:
Perform patient education regarding medications prescribed by Toledo Clinic specialties including, but not limited to, Oncology, Rheumatology, Endocrinology, Urology, and Neurology Dispense oral and IV Oncology medications prescribed by licensed health care providers.
Principal Duties & Responsibilities:
1. Educate patients about his/her drug therapy
2. MTM/Medication reconciliation
3. Symptom management consultation
4. Work in conjunction with UTMC staff for 340B program compliance
5. Dispense oral and IV oncology medications per diem
6. Maintain adherence to policies and procedures in accordance with USP 797/800
7. Maintain quality control and adherence to all regulation requirements, licensing and dispensing laws
8. Oversee drug inventory
9. Screen medication orders for dose modifications/prospective DUR
10. Oversight of usage of 340B inventory
11. Calculation of drug dosing based on clinical parameters
12. Occasional travel to various Toledo Clinic facilities (Toledo, Sylvania, Maumee)
13. Other duties as assigned
Knowledge, Skills & Abilities:
Required:
- Maintain Ohio pharmacist license.
- Adheres to, and ensures department's compliance with, the clinic's policies and procedures.
- Maintains awareness of the department's budget at all times and works within the budgetary figures.
- Interprets and reviews financials and offers explanation when expenses or liabilities are excessive.
- Establishes departmental goals and completes goals within established time frame.
- Demonstrates adaptability to expanded roles.
- Adheres to all clinic policies and procedures
Education:
- Bachelor of Science in Pharmacy.
Preferred:
- Previous 340B experience
- MTM experience
- Prior IV room experience
- Prior oncology experience
- Doctor of Pharmacy degree
