The Ambulatory Clinical Pharmacy Specialist is an embedded member of the care team in the ambulatory clinic practice, e.g., rheumatology and oncology. Primary responsibility is to ensure the provision of safe and effective medication therapy, inclusive of monitoring adherence to improve outcomes. The Ambulatory Clinical Pharmacy Specialist is expected to possess specific disease state expertise in their respective practice area. They will make recommendations to the interdisciplinary team to assure that the drug therapy is optimal for the individual patient’s needs. The Ambulatory Clinical Pharmacy Specialist services include but are not limited to assessing patient needs, include disease specific characteristics into medication therapy, adjusting care according to patient response, and providing appropriate clinical interventions to detect, mitigate, and prevent medication adverse events. The Clinical Pharmacist is expected to provide medication and practice-related education/training and actively serve as preceptors for doctor of pharmacy students and residents.
