Clinical Pharmacist

Job Description
Flexible Pharmacy Staffing is seeking a Clinical Pharmacist for an interim Health Clinic non-dispensing assignment in Clackamas County, Oregon (just south of Portland). Position starts overlap training ASAP and is anticipated to go for about 3 months (approximately October/November, or when the medical leave ends). Interviews will begin next week.

Schedule Details:

-Flexible Remote/office 50/50 hybrid weekday schedule, up to 32hours/week

Project Start Date Notes:

- Start training ASAP, then work solo for several weeks. Possibility for continuing opportunities for PTO and on-call coverage.

Clinical Pharmacist Duties:

-No filling of prescriptions
-Assist with PA's
-Execute CDTM protocols
-Work with Care Teams
-May rotate among clinics, including in-person consultations

Beneficial Experience for the Clinical Pharmacist Position. Please don't be shy about expressing interest if you don't match all of these criteria

-Ambulatory Care Residency

Experience with EPIC software

-Epic is usually found in hospital settings but is found more and more in ambulatory pharmacy settings
-Experience working with Care Teams
-Experience with Prior Authorization Processing

Experience with CDTM (Collaborative Drug Therapy Management) Agreements

-Diabetes
-Hyperlipidemia
-Hepatitis C
-Smoking Cessation
-TBD (5th CDTM)

ESSENTIAL PHYSICAL FUNCTIONS:

- Light to moderate physical effort (lift/carry up to 25 lbs.) -Heavier weights with assistance
- Repetitive motions and/or prolonged computer use
- Working at a fast pace, subject to many interruptions and both physical and mental stress
- All jobs currently require clear and concise verbal and written communication skills in the English language.
