The University of Illinois Chicago College of Pharmacy, Department of Pharmacy Practice is seeking a full time, motivated professional for the position of Clinical Faculty (Open Rank)/Clinical Pharmacist who will be assigned to the Telemedicine Diabetes Clinic.

Duties:

The ideal candidate will be responsible for providing pharmaceutical care to patients in the area of diabetes management. The pharmacist will join a team of physicians and advanced practice nurses to  provide care to patients in an Illinois Department of Corrections (IDOC) facility via a telehealth model.  This new service will mimic the already existing model for HIV and Hep C care for patients in these facilities.   The pharmacist will work from the University of Illinois Hospital & Health Sciences System (UI Health) campus located  approximately 2 miles west of downtown Chicago.  UI Health comprises a clinical enterprise that includes a 462-bed academic hospital, 26 outpatient clinics and 13 Mile Square Health Center facilities.  It also includes the seven UIC health science colleges.  The hospital and clinics serve an urban patient population. 

 

The new hire will be assigned to the IDOC diabetes telemedicine clinic 4 days per week with 1 office day per week. Hours will be Monday – Friday during the day.  The clinical pharmacist will  work under a hospital approved protocol to participate directly in the assessment and management of patients with diabetes.  The pharmacist will recommend and implement an evidence-based care plan in collaboration with the treating credentialed practitioner, resident or fellow; initiate, modify, or discontinue drug therapy for treatment of diabetes or accompanying hypertension and/or dyslipidemia to achieve therapeutic goals; and order laboratory tests to assist in the management of diabetes or accompanying hypertension and/or dyslipidemia or the medications used to treat these diseases.  The candidate will also be responsible for nurse, physician, and pharmacy staff development; coordinating and conducting quality initiative projects and developing collaborative treatment and research protocols.

 

The academic responsibilities of the clinical faculty member (Clinical Assistant/Associate/Full Professor) include the provision of didactic lectures and precepting entry level Doctor of Pharmacy students, PGY1 residents and PGY2 residents. Scholarly activities, including presentations and publications, are an expectation as well.

 

Minimum Required Qualifications:

  • Candidates must be licensed or eligible for Illinois licensure as a professional pharmacist at time of employment. Candidates must be a graduate of an ACPE accredited College of Pharmacy PharmD program. Candidates must have completed an ASHP accredited PGY1 residency training. PGY2 Ambulatory Care residency or 2-3 years of equivalent clinical experience as a clinical pharmacist managing diabetic patients is required. Candidates must possess knowledge and skills such that they qualify for a non-tenured clinical faculty position with the Department of Pharmacy Practice at the UIC College of Pharmacy. Academic rank commensurate with experience.

 

Preferred Qualifications and Skills:

  • Board certification is recommended. Highly preferred strong clinical knowledge base; analytical and problem solving skills; strong verbal and written communication skills; ability to consistently meet deadlines; demonstrate flexibility and ability to work independently and adapt to a changing work environment; excellent planning, time management and organizational skills.
