Job DescriptionThe Clinical Pharmacy coordinator is responsible for leading and managing comprehensive clinical pharmacy services for Pharmacy practice sites. Is responsible for central clinical pharmacy support through participation in and /or management of standard department programs and services, and participation in the development of organization-wide drug therapy management materials and tools. Is the Pharmacy Expertise for the Antibiotic Stewardship Program.
Other key responsibilities of this position include, but are not limited to: staff development of practice related policies /procedures and guidelines which promote medication safety, appropriate use of medications, formulary compliance, cost-effective practice and optimal patient outcomes
Position Responsibilities:
Performs all principal duties and responsibilities of a clinical pharmacist coordinator. Assists Pharmacy management in maintaining the organization's clinical pharmacy program and ensure it provides patient-centered, cost effective, and safe care including the development of new clinical pharmacy programs and services.
Develops policies, protocols and guidelines that support the creation and maintenance of the clinical pharmacy program, ensuring compliance with Centers for Medicare and Medicaid Services (CMS), Drug Enforcement Agency (DEA), Department of Public Health (DPH) and The Board of Registration in Pharmacy and other regulatory standards.
Provides support to the Medication Safety Committee and other hospital committees as applicable.
Reviews and trends medication events, works collaboratively to develop and implement error reduction and mitigation strategies
Develops, implements, and monitors process improvement initiatives to ensure safety in pharmacy and medication use.
Provides educational information/seminars on medication safety to physicians and other Licensed Independent Practitioners, nursing staff and other allied health care professionals as needed.
Precepts pharmacy students as applicable.
Performs other related duties as assigned.
Qualifications:

Qualifications:
- Bachelor's degree in Pharmacy required. Doctoral degree in Pharmacy (PharmD) preferred
- Licensed Massachusetts Pharmacist required
- Board Certification in Geriatrics required
- ASP certificate through an accredited program required
- 1-3 years institutional or LTCH experience
