Job Summary

The Solid Organ Transplant (SOT) Clinical Pharmacist Specialist is an essential member the SOT multidisciplinary program at UT Southwestern (UTSW) Medical Center.

The SOT clinical pharmacist specialist responsibilities include:

  • Manages clinical activities, priorities, and problems, pharmacist staffing, and workloads. 
  • Provides status report to pharmacy management, effectively communicating issues, concerns, and suggestions as appropriate.
  •  Manages and develops clinical initiatives and determines therapeutic alternatives when drug shortages are identified, and communicates with medical staff leadership and key stakeholders. 
  • Assists with budget preparation and daily financial activity specifically in relation to high cost drugs. 
  • Assists the Acute Care Clinical Manager and Director with meeting administrative goals and objectives. 
  • Facilitates and ensures a safe transition of care to discharge from the hospital for new SOT patients.  

Duties performed may include one or more of the following core functions:

 

  • Participation in multi-disciplinary rounding, clinic discussion, IRB, and/or service/line meetings
  • Manage clinical activities, priorities, and problems, pharmacist staffing and workloads
  • Provides status report to pharmacy management effectively communicating issues, concerns, and suggestions as appropriate
  • Assists with budget preparation and daily financial activity specifically in relation to high cost drugs
  • Assists the Director and Assistant Vice President of Pharmacy with meeting administrative goals and objectives.

 

Experience and Education

  • Both PGY1 and PGY2 from an ASHP-accredited residency program; PGY1 from an ASHP-accredited residency program with 5 years of progressive clinical practice within specialty; completion of specialty fellowship within specialty; or 8 years progressive clinical practice experience within specialty.
  • Bachelor of Science (BS) degree or Doctor of Pharmacy (PharmD) degree from an ACPE accredited school of pharmacy.
  • Eligible for licensure by Texas State Board of Pharmacy (TSBP) is required and Texas license with TSBP within 30 days of date of hire. 
  • Current Basic Life Support (BLS) certification with course accredited by the American Heart Association (AHA).
  • Basic computer skills and data entry. English (speaking, reading, writing). Good general health and stress coping ability.
  • Moderate physical effort: prolonged standing/walking, occasional lifting/carrying (up to 25 lbs.), reaching, bending, crouching, etc. OSHA Category III.  

Job Duties

  • Performs job responsibilities in accordance with organizational and departmental policies and procedures
  • Performance improvement objectives and environmental/infection control standards.
  • Develops/implements process to meet/comply with the Joint Commission and regulatory requirements.
  • Coordinates staff training, competencies, certification and continued education specific to specialty.
  • Assists clinical staff pharmacists with therapeutic monitoring, patient assessments, interpretation of lab values, and pharmacokinetic calculations.
  • Identify opportunities for process improvements, develops and implements policy and procedures for prompt, safe and efficient patient care. 
  • Required clinical activities include a)
    • Yearly development of a cost-saving initiative, quality, and/or operations improvement with manager sponsor, MUE or drug class review with manager sponsor 
    • Yearly ACPE/non-ACPE presentation (rotating between ACPE/non-ACPE)
    • Individual CE in areas of specialty (6 hours) 
    • P&T or Subcommittee Attendance (50% Attendance requirement) 
    • Annual submission of research project on Research Submission Template (Due April 1st every year).
  • Actively promotes pharmacy.
  • Coordinates and participates in multidisciplinary in-services/meetings, publishes articles (i.e. staff newsletter), individualized drug information, rounds, consults, etc.
  • Maintains high visibility/availability through participation in in-services, meetings and daily activities. Participates in the education/experiential training of pharmacy students and pharmacy residents.
  • Actively manage P&T initiatives through monitoring of documented interventions.
  • Verifies data, develops measures and reports outcomes (Performance Improvement).
  • Supports cost savings initiatives.
  • Has active role in assessment and selection of cost-effective pharmaceuticals.
  • Reviews formulary addition request and develops monographs for presentation.
  • Manage the formulary system and procedures for acquisition of non-formulary product.
  • Consistently works and problem solves independently and effectively.
  • Takes charge when necessary and is looked to for direction.
  • Makes the right decisions and uses good judgment.
  • Cross trained and multifunctional maintaining competency in multiple areas.
  • Mentors clinical staff pharmacists and supportive personnel.
  • Delegates appropriate/reasonable tasks and evaluates performance.
  • Represents Pharmacy Department at P&T Committee and Service Line Meetings
  • Performs computer order entry in an accurate, thorough and timely manner.
  • Reviews/updates medication profiles (therapies, allergies, height, weight, etc) and is cognizant of patient charges.
  • Reviews medication orders for therapeutic appropriateness, duplications, interactions, overall effectiveness and safety.
  • Seeks clarification if necessary and documents interventions (i.e. ADRs).
  • Actively enhances personal growth and development by participating in educational programs, literature searches, in-services and meetings. Actively participates in new employee orientation and training and evaluations.
  • Duties performed may include one or more of the following core functions:
    • Directly interacting with or caring for patients
    • Directly interacting with or caring for human-subjects research participants
    • Regularly maintaining, modifying, releasing or similarly affecting patient records (including patient financial records)
    • Regularly maintaining, modifying, releasing or similarly affecting human-subjects research records
  • Maintains patient confidentiality in accordance with University Medical Center policy. 
  • Consistently maintains control of feelings, emotions, or body language. 
  • Demonstrates through vernal and non-verbal communications openness to change or new ideas.

  • Supports teamwork and cooperation with co-workers in daily activities. Readily volunteers assistance to help co-workers complete tasks.

Work is performed primarily in William P Clements University Hospital, Zale Lipshy University Hospital or UTSW Ambulatory Clinics. Security
