Clinical Pharmacy Specialist - PACT
The Patient Aligned Care Team (PACT) Clinical Pharmacy Specialist (CPS) is primarily responsible for providing and documenting pharmaceutical care of all assigned Veterans within PACT. This includes taking responsibility for outcomes associated with medication use, referral of requests to the appropriate level of care, process improvement, and functioning at the highest level of clinical practice.
