The direct patient care pharmacist integrates as a member of the care team within the physician practice. The pharmacist is responsible to manage patients' drug therapy to ensure optimal, cost effective care delivery and improve health outcomes. This role may involve multiple clinic locations.

If you are located in Orlando FL, you will have the flexibility for a hybrid telecommute* schedule as you take on some tough challenges.. This role requires some remote work and some on site clinic visits. This role will cover South Orlando (Polk and Osceola Counties)

Primary Responsibilities:
  • Performs comprehensive medication reviews, utilizing MTM platforms where feasible
  • Works collaboratively with providers to adjust drug therapy as allowed per state regulatory agencies
  • Participates in clinic team huddles, meetings, and patient discussions
  • Consults with provider in the management of chronic disease states
  • Provides disease state education for patients and teammates on appropriate use of medications and self-management individually and in a group setting
  • Provides post-discharge medication reconciliation for all transitions of care
  • Documents all activity within clinic EHR and other required tracking systems
  • Supports CMS Stars quality measures for clinic and Medicare Advantage programs
  • Implements population health strategy at the patient level
  • Other duties as assigned

Required Qualifications:
  • B.S. OR PharmD from an ACPE-accredited School of Pharmacy
  • Current active license in good standing with appropriate state Board of Pharmacy

Preferred Qualifications:
  • Additional certification/licensure
  • 2+ years of experience as a practicing pharmacist or completion of a residency program
  • Experience in managing drug therapy for patients with chronic conditions
  • Competent at the intermediate level or higher with MS Office programs such as Word, Excel and Power Point
  • Bilingual (English/Spanish)
