Optum New Mexico is seeking a full-time Clinical Pharmacist to join our team in beautiful Kansas City. At Optum, we are transforming healthcare nationally while providing Physician-led care locally.

Primary Responsibilities:
  • Oversee and perform process of medication reconciliation within 30-day time frame of Medicare Advantage patient discharge from an inpatient facility
  • Emphasis on providing communication with PCP to resolve any discrepancies
  • Evaluate MA patient medications for appropriateness, safety, effectiveness, adherence and cost
  • As time permits, performs in depth poly pharmacy evaluations, indications, renal/hepatic dosing, adherence, drug interactions
  • Assists with development of new program building
  • Collaborate with other care providers in the effective sharing of practice expertise. Communicate clinical information timely, accurately, thoroughly and appropriately to other providers of patient care. Review medication regimens; adjust medications based on protocol, give recommendations of medication therapy to prescribers.
  • Work providing direct patient care to patients in clinical areas as identified. Work includes review of medication regimens, adjustment of medications based on protocols, recommendations of medication therapy to prescribers. Additional patient education and support is expected.
  • Document clinical care in a timely manner. Code and bill accurately per established protocol and procedure. Ensure that records are detailed and complete, and comply with related standards and regulations.
  • Future opportunity to participate in chronic disease state management for endocrine diseases, hypertension and heart failure.

Required Qualifications:
  • Current Kansas/Missouri Pharm. D License or acquired prior to start
  • 3+ years clinical pharmacy experience in working face to face with physicians and health care providers in hospital or ambulatory care
  • Full COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements. UnitedHealth Group will adhere to all federal, state and local regulations as well as all client requirements and will obtain necessary proof of vaccination, and boosters when applicable, prior to employment to ensure compliance. Preferred Qualifcations:
  • Completion of an American Society of Health Systems Pharmacists (ASHP) Accredited Residency
  • Experience in providing patient care under a Global Risk/Medicare Advantage Model
  • Fluency in EMR systems & technology
