Job Title: Clinical Pharmacy Specialist, Pain Management

Department: Pharmacy

FLSA Status: Exempt

Position Summary: The Clinical Pharmacy Specialist for Pain Management Services is responsible for provision of pharmaceutical and consultative care services for patients with pain syndromes in both inpatient and ambulatory settings.

Essential Functions of the Job:
  • Maintains collaborative and comprehensive medication therapy management clinics and services in designated areas of service, under a scope of practice;
  • Participation in patient education; evaluate, monitor and modify drug therapy; provide recommendations to optimize patient outcomes; minimize safety issues; ensure cost effectiveness; and provide drug information;
  • Introduction and implementation of Pain management and Opioid stewardship program, policies & protocols and performance improvement related to area of specialty;
  • Ongoing monitoring of all patients for appropriate response, adverse drug event monitoring and reporting;
  • Coordinates training and education of pharmacy staff and students
  • Design, implement, assess, monitor and document therapeutic plans utilizing the most effective, least toxic and most economical medication treatments;
  • Help achieve positive patient centered outcomes (e.g., establishing and progressing towards individualized function-related goals, improving quality of life, minimizing theoretical risk of drug-related harm) through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas;
  • Implements risk mitigation strategies and review of laboratory and other tests to help determine efficacy and toxicity of medication therapy.
  • Manage medication therapy by initiating, modifying and discontinuing medication both independently and in collaboration with other practitioners when needed
  • Provide pharmaceutical care to patients in direct care setting to achieve therapeutic goal and make referrals as needed to optimize patient outcomes and treatment success.
  • Participate in committees and task forces including but not limited to pain, palliative and supportive care, sedation, transitions of care, and pertinent pharmacy and therapeutics committee or subcommittees.
  • Provides professional consultative services related to their designated area of practice, to other health care providers. Professional consultative services include but are not limited to extensive drug information services, formulary management, and medication recommendations.
  • Functions independently in day to day clinical and administrative duties associated with the position, consulting with pharmacy management only when required. In general, uses a high degree of professional judgment to solve complex problems which routinely arise throughout the work day.

  • Requirements:

    Education: Doctrate of Pharmacy required. Pharmacy residency strongly preferred.

    Skills: Strong communication, project management, primary literature evaluation

    Experience: 1+ years Hospital Pharmacy, Clinical experience

    Certification/License: License to dispense Pharmaceuticals in the State of New Jersey
