Clinical Pharmacist
Location: Scottsdale, AZ
Type: Contract to Hire (FTE conversion at 3 months)
Looking for a Clinical Pharmacist with a passion for developing population health solutions that improve health outcomes. The Clinical Pharmacist is primarily responsible for providing patients and providers with therapeutic interventions that improve chronic conditions and outcomes with a particular focus on members with Diabetes and pharmacological challenges such as high-risk medications and polypharmacy.
This individual works closely with our network of primary care physicians, care managers and other clinical staff, as part of a clinical support team focused on identifying pharmacological risk, patient outcomes and managing avoidable costs. The Pharmacist serves as the pharmacological subject matter expert and provides consultative support to the Care Management team and our network providers.
Education/Experience:
Active and current licensure by the Arizona State Board of Pharmacy

Education/Experience:
- Doctor of Pharmacy Degree (PharmD) from university or college of pharmacy accredited by the Accreditation Council for Pharmacy Education (ACPE)
- Minimum two (2) years of work experience as clinical pharmacist or equivalent training through a residency program PGY1
Active and current licensure by the Arizona State Board of Pharmacy
