Job Details
The Pharmacy Department has a full-time opportunity for a Clinical Pharmacist in Critical Care at Atrium Health-Union, a non-profit, 182-bed hospital in Monroe, NC providing comprehensive emergency services and specialty medical care. The campus offers a variety of medical services, including a day surgery center, cancer treatment center, long-term care facility, behavioral health center, specialty care clinics, a community wellness and outreach program, and women and children’s center, interventional heart program and physician practices.
Job Summary
Oversees the provision of safe and effective medication therapies through collaboration with the patient’s care team. Reviews provider medication orders for appropriateness. Assists with the strategic planning and implementation of pharmacy initiatives for the Division of Pharmacy and serves as a clinical leader in designated area.
Responsibilities:
Rounds daily with the multidisciplinary critical care patient care team.
Serves as clinical educator to pharmacists, providers, nurses, and other members of the healthcare team as appropriate.
Facilitates transitions of care through discharge medication list review and intervention.
Uses effective patient education techniques to provide counseling to patients and caregivers.
Serves as preceptor for pharmacy students and residents.
Mentors, trains, and assists with the performance evaluation of pharmacists.
Retrieves and evaluates relevant biomedical literature, formulates an appropriate response, and provides concise and timely response to drug information requests.
Collects information needed to design medication therapy regimens.
Designs, recommends, monitors, implements and documents patient specific pharmacotherapeutic regimens.
Participates in the management of medical emergencies
Collaborates with pharmacy colleagues across the Division of Pharmacy to manage the formulary and the medication-use process. Assists in the acquisition of non-formulary, compassionate use, and investigational medications.
Performs quality assurance and performance improvement of the medication use system.
Qualifications:
Doctor of Pharmacy Degree from an accredited College of Pharmacy
Completion of a PGY-2 specialty residency in critical care pharmacy is preferred
If no PGY-2 residency, at least 5 years of experience as a critical care pharmacist preferred.
Skills in multi-disciplinary team involvement with direct patient care competency in critical care drug therapy desired
Required License/certifications:
North Carolina Pharmacist License, or eligibility to sit for the exam
Board Certified Cardiology Pharmacist (BCCP) preferred
Basic Life Support (BLS) required
