The University of Illinois Chicago (UIC) College of Pharmacy seeks a Clinical Faculty (Open Rank)/Clinical Pharmacist who will participate in the didactic and clinical programs of the Colleges of Pharmacy and Medicine located in Rockford, Illinois. The successful candidate will play an integral role on the UIC Health Sciences Campus in Rockford through didactic teaching, scholarly activity, and the precepting of pharmacy/medical students and residents in the clinical setting.

 

The position is shared between the University of Illinois’ Colleges of Pharmacy and Medicine at Rockford, with faculty appointments in both the Department of Pharmacy Practice and Department of Family and Community Medicine. The faculty member will teach medical and pharmacy students in the classroom and will serve as a course director for one of the courses in the first two years of medical education. They will work with a variety of faculty to inform the curriculum of both Colleges.  The clinical practice site is at the nearby Miles Square Health Center - L. P. Johnson Rockford. This FQHC trains pharmacy and medical students, the college of medicine’s family medicine residents, and the pharmacy PGY1 resident. The faculty member will teach, precept students and residents, and work as part of the clinics interprofessional team to provide patient care, therapeutic drug monitoring, and education to patients and their caregivers to ensure safe and effective outcomes. Additionally, they will round with family medicine residents on hospitalized patients and provide seamless transitions of care to the outpatient setting. Diverse patient populations served include those with diabetes, heart disease, hypertension, lipid disorders, asthma, COPD, mental health and substance use disorders, and other chronic disease states. Additional duties include service on college and clinic committees related to the clinic processes and family medicine residency program, adopting and implementing evidence-based guidelines for chronic disease states, mentoring residents on medication related research and quality improvement projects, and conducting scholarly activities resulting in presentations and publications.

 

Minimum requirements for this position include: PharmD degree from an ACPE accredited College/School of Pharmacy plus completion of a PGY1 residency or equivalent experience, PGY2 residency training is preferred. Candidate must possess knowledge and skills to qualify for an academic faculty position (level commensurate with experience) in the Colleges of Pharmacy and Medicine. Candidates must present evidence of, or eligibility for, Illinois licensure as a registered pharmacist by start date.
