The Clinical Pharmacist works collaboratively with Physician teams, advanced practitioners, medical residents and staff to promote and improve safe medication use. Provides patient specific medication use teaching in high risk patients (i.e. polypharmacy, pediatric patients, patients on high risk medications or are at risk for readmission due to medication non-compliance). Assist with drug therapy management in medical emergencies (i.e. code, trauma, stroke).
  • Participate in the development, implementation and monitoring of evidence based, medication use guidelines that meet National criteria for quality measures such as in Heart Failure, Stroke, AMI, Pneumonia and Sepsis. Participates in or leads quality improvement, medication safety and emergency preparedness initiatives as needed. Promote and demonstrate rational, cost-effective drug therapy through effective, criteria based management of high cost therapies.
  • Serve as a drug information resource and pharmacy liaison for physicians, residents, patients and staff in appropriate units or departments, to ensure the timely administration of appropriate medications and resolution of other issues as needed. Serve as a pharmacy representative to other Carle Hospital Committees as they relate tospecialty practice.
  • Perform order entry, order verification and clinician order review as required while documenting clinical interventions and consultations following the requirements of the Department of Pharmacy for documentation. Provide pharmacokinetic consultations and other dosing recommendations as required. Performs other duties as assigned.
  • Provide Medication Therapy Management Services as defined in the Illinois Pharmacy Practice Act while keeping informed of all local, state and regulatory laws or requirements. Maintains awareness of contemporary trends in the profession of pharmacy through the professional literature and attendance at professional organization meetings, institutes and/or seminars.
  • Assists in the development, implementation, and evaluation of aSpecialtyPGY1 Pharmacy Residency rotation. Meets the requirements to be a Residency Preceptor. Precepts residents.
