A clinical pharmacist provides patient care through the provision of direct operational and/or patient care services. They also collaborate within the department to enhance operations and clinical programs. Clinical pharmacists ensure the safe and appropriate use of medications through the provision of drug information, drug therapy monitoring, distributive and supervisory pharmacy services within the framework of hospital and pharmacy policy, standard practices and state/federal laws. They must also meet departmental staff development requirements. A clinical pharmacist will participate in departmental or hospital committees, actively participate in the provision of education, and promote the profession. Provision of education can include members of the interdisciplinary team (physicians, mid-level providers, nursing, social work etc), patients, the pharmacy department as well as pharmacy students and residents.

The Ambulatory Clinical Pharmacist collaborates with the multidisciplinary team to improve care, achieve optimal outcomes, and provide continuity of care to patients through the provision of pharmacy patient care services to include therapy care plan development, medication management, education, monitoring and follow up. In order to enhance collaborative patient care, pharmacists will be delegated the authority to obtain a patient medical history, order and evaluate laboratory results, conduct limited physical examinations and make medication decisions including modifying or discontinuing treatment when appropriate with their scope of practice.

Patient care services provided by the pharmacist under the supervision of the physician:

  • Obtain patient medical history
  • Order and evaluate laboratory results
  • Order and evaluate diagnostic tests for preventive care
  • Conduct limited examinations
  • Make medical decisions including modifying or discontinuing therapy
  • Recommend referrals, including but not limited to eye or dental exams & vaccines
  • Medication refill authorization

Documentation: All patient encounters will be documented in the patient record and will be available to the supervising physician for review

ESSENTIAL DUTIES & RESPONSIBILITIES:

  • Provides pharmaceutical excellence through direct patient care activities as a member of the multidisciplinary healthcare team. 55%
  • Provides accurate medication order-entry, drug distribution, and patient care (which may include point of care testing). Confirms the appropriateness of drug therapy as it relates to the care of specific, specialized patient populations. Seeks to minimize turn-around-time for products and services. 5%
  • Actively participates in department and institutional committees and projects. 10%
  • Actively participates in the provision of health education including didactic and experiential training programs as well as education for Prisma nursing, pharmacy and medical staff. 10%
  • Provides concise, applicable, timely responses to requests for drug information. 5%
  • Acts as liaison between pharmacy and assigned patient care area solving distribution problems and monitoring drug supplies for proper storage, etc. 15%
     

MINIMUM JOB REQUIREMENTS:

  • Minimum education: Bachelor’s degree
  • Graduation from an ACPE-approved college of pharmacy, PharmD preferred
  • Minimum experience: 1 year
  • Type of experience: Completion of an ASHP-Accredited PGY1 Residency Required (or 5 years equivalent experience)

REQUIRED KNOWLEDGE, SKILLS & ABILITIES:

  • Display knowledge of medical and pharmacy terminology; possess good mathematical & computer skills
  • Understand SCBOP/DHEC/DEA rules/regulations
  • Be a problem solver; able to work in dynamic interdisciplinary team situations; handle urgent, stressful conditions.
  • Exhibit excellent interpersonal skills in dealing with subordinates, peers, supervisors and others outside the department.
