Job Summary: 

Provide appropriate recommendations for drug therapy and contribute to the optimal medication management of patients. To review, interpret, clarify, verify, dispense, and compound medications and other pharmaceutical supplies pursuant to a medication order.

Detailed Description:

Participate in regular rounds to patient care areas to assist with issues of medication usage. Complete daily medication use evaluations for the patients on assigned team/area. Provide appropriate patient specific medication recommendations that ensure safety, efficacy, and cost effectiveness.

 

Demonstrate ability to review and verify medication orders to assure appropriateness utilizing clinical information in the electronic medical record. Prioritizes medication orders based on acuity and order status (i.e. STAT, NOW, routine).

Assist in education and training of pharmacy students and pharmacy technicians, and residents.

 

Initiate and assist in protocol development and implementation in specialty practice area to optimize medication utilization, improve patient outcomes and improve cost effectiveness.

Participate in multi-disciplinary committee meetings, research, drug utilization reviews, medication class reviews, formulary medication requests, staff education, presentations, and in-services as requested or needed by the department or hospital.

Serve as a resource for the pharmacy team, nursing, and prescribers for questions/issues related to pharmacotherapy; analyze literature to support usage and contact prescribers as needed.

Compound and dispense pharmaceutical preparations which includes, but are not limited to compounded sterile products (IV solutions, injectable medications, and irrigation solutions), oral products (solutions, suspensions), and topical preparations (ointments, creams).
