What you will be doing

This position is 100% on-site at Reading Hospital, located in West Reading, Pennsylvania. Typical work hours are M-F between 8 AM and 5 PM, with infrequent work on weekends.

This position is based primarily in a neurology clinic servicing a variety of disease states (migraine, seizure, movement disorders, multiple sclerosis, etc.) however may also assist in infectious disease clinic as needed. Pharmacist will collaborate directly with patients and other healthcare providers. A desire to work in a clinical pharmacy position with a strong understanding of retail pharmacy operations and prescription processing is optimal to succeed in this role.

PRIMARY DUTIES AND RESPONSIBILITIES:
  • Demonstrate value of ambulatory pharmacy partnership to physician practices.
  • Maintain presence in clinic, field medication questions from providers, such as formulary product selection, product dosing, ADR's, and other considerations.
  • Ensure prescriptions flow through ambulatory pharmacy and are identified by pharmacy staff.
  • Maintain patient list, schedule appropriate follow-ups to assess medication safety and efficacy, communicate treatment plan recommendations with provider.
  • Coordinate patient refills. Ensure auditable records showing proof-of-delivery are maintained.
  • Prepare and submit prior authorizations, manage denials/appeals.
  • Triage prescriptions which are limited distribution drugs or out-of-network.
  • Maintain patient schedule for clinical pharmacist services including, but not limited to, medication counseling, pillbox refills, immunization, medication administration.
  • Act as financial advocate for patient when needed, leverage charitable foundations, state programs, and/or manufacturer programs to ensure medication affordability.
  • Demonstrate ability to navigate various platforms and document appropriately in EMR and specialty therapy management platform.
  • Cross-train to ambulatory pharmacy, assist with staffing needs, as required. This may include weekend and/or holidays per business needs.
  • Contribute to expansion of ambulatory pharmacy services through active participation in navigator committee and projects.


What your background should look like

  • PharmD degree required.
  • PGY1 residency or previous experience in specialty pharmacy is highly desirable.
  • Unrestricted pharmacist license (incl. immunization) in the state of PA is required. If out of state, must obtain licensure within 90 days of receiving offer letter.
  • Experience with 340B purchasing program is beneficial.


MINIMUM SKILLS, KNOWLEDGE AND ABILITY REQUIREMENTS:
  • Must be proficient in Microsoft Office, including Word, Excel, and PowerPoint.
  • Experience in Epic or similar EMR platforms.
  • Experience with retail pharmacy prescription processing and claims adjudication.
  • This role involves working with broad array of complex disease states and medical conditions; candidates should be comfortable working with clinical data, lab values, and reading/writing notes in patient medical records.
