didgʷálič Wellness Center has a great opportunity for a Clinical Pharmacist to join an integrated care clinic that combines primary care, behavioral health, and opioid treatment under one roof. The position provides pharmaceutical care services including medication reconciliation, medication management, and therapeutic drug monitoring. The Clinical Pharmacist will actively participate in establishing guidelines and protocols for additional clinical pharmacy service programs and related disease state management, while also practicing under a Collaborative Drug Therapy Agreement to prescribe and maintain medications for opioid use disorder. Responsibilities:
  • Provides clinical pharmacy services support and drug information services.
  • Works with patients and staff by reconciling medication lists, giving pharmacological information to a multidisciplinary health care team, and extensively monitoring patient drug therapies.
  • Performs Drug Utilization and Drug Interaction Reviews for patients.
  • Mid-Level Practitioner for prescribing and managing medications for opioid use disorder (MOUD) under Collaborative Drug Therapy Agreement with Chief Medical Officer.
  • Ensures that pharmacotherapy treatment recommendations in the opioid treatment program are consistent with protocol initiatives, standards of care and regulations
  • Play a leading role in staff and patient vaccination programs including quality improvement, population management, assessment of individual vaccine needs and follow up.
  • Serves as a clinical resource and consultant for medication utilization, medication management, pharmacy quality initiatives and safety.
  • Reviews medication profiles and monitors for compliance or potential abuse and participates in resolution when problems are identified.
  • Presents and coordinates educational programs for the medical staff.
  • Participate in ongoing reviews of medication policy and procedures, protocols, and guidelines to improve utilization and management
  • Assists in the development and implementation of clinic protocols for related disease states.
  • Helps develop policies and procedures to support the clinical guidelines in chronic care management.
  • Provides oversight of dispensary if necessary in the absence of the Pharmacy Services Manager.


What We're Looking For:
  • Valid state pharmacist license (or must meet education requirement(s) for state licensure).
  • Completion of ASHP residency (PGY1) or 2 years clinical experience in ambulatory care preferred.
  • Degree from an accredited pharmacy college recognized by the American Council of Pharmaceutical Education (ACPE).
  • Current BLS for Healthcare Provider CPR or CPR/AED for the Professional Rescuer certification.
  • Vaccine administration experience preferred.
  • Ability to work in a cross-culture environment, · Effective communication; written, verbal, and interpersonal skills, including conflict resolution.
  • Basic computer and software experience using MS Office Suite.
  • Ability to pass extensive criminal background check, pre-employment drug and provide proof of COVID-19 vaccination.
