The Pharmacy Department has a full-time opportunity for a Clinical Pharmacy Specialist in Emergency Medicine at the Jamaica Hospital Medical Center. The ideal candidate will be responsible for provision of pharmaceutical care to outpatients managed by the Emergency Department Clinicians and admitted patients in the Emergency Department (ED). The Jamaica Hospital Medical Center Emergency Department is a Level 1 Trauma Center and Thrombectomy-Capable Stroke Center which treats over 117,000 patients annually.  We also offer the following Emergency Medicine services: pediatric emergency department, geriatric emergency department, Fast Track center, chest pain center, and a cardiovascular interventional suite. Activities include the assessment and care of ED patients: evaluate, monitor, and modify drug therapy; provide recommendations to optimize patient outcomes and medication verification, participation in procedures utilizing high risk medications, and resuscitations.

The pharmacist will also work to develop and implement policy and procedure and medication use protocols or guidelines as they pertain to ED patients including optimization of effectiveness, minimization of safety issues and adverse events, and insuring cost effectiveness.

The pharmacist should be an effective communicator and be able to integrate themselves within the health care team and current structure of the pharmacy department as well as document clinical activities. Participation in pertinent Hospital Committees.

Responsibilities:

  • Participate as a member of the multidisciplinary Emergency Department team to provide pharmacotherapeutic (including pharmacokinetic monitoring) management for all current Emergency Department and admitted patients
  • Provide pharmacy review of high-risk medication orders prior to administration
  • Identify adverse drug events (ADEs) and assist in their management, prevention, and reporting and develops process improvements to reduce drug errors and preventable ADEs
  • Provide patient-specific medication use teaching for discharge medications when appropriate
  • Focus on cost avoidance and cost savings due to medication use in the ED
  • Facilitate proper information transfer with regard to medication use for patients converted to inpatient registry from ED
  • Work collaboratively with other clinicians and health care providers to implement and maintain innovative disease management programs and clinical pharmacy services
  • Participate in the development of medication management programs within the institution, including clinical guidelines, critical pathways, disease management, and drug use programs
  • Oversee the investigational drug program of the hospital in relation to Emergency Medicine
  • Participate in the professional development and competency of clinical staff. Function as an educational resource for pharmacy staff
  • Keep informed of all local, State, and Federal laws covering the storage, handling, and dispensing of drugs; and interpret each prescription order to determine that it meets all legal requirements
  • Keep informed of the actions, side effects, and proper use of all new drugs as they are made commercially available, as well as of all investigational drugs being studied at this institution
  • Maintain awareness of contemporary trends in the profession through the professional literature and regular attendance at professional meetings, institutes, and seminars
  • Participate collegially in the development of new programs, services, and practices in the education activities of the department and the management and administration of the department
  • Documentation of all activities/interventions for cost savings and outcome purposes
  • Actively participates on the Critical Care Committee, Code PI Committee, Antibiotic Stewardship, and the Pharmacy, Therapeutics, and Nutrition Committee (P,T&N). Participates in the Performance Improvement Committee and drug use evaluation program as requested
  • Perform formulary evaluation and class reviews for emergency and intensive care related medications and present to the P,T&N Committee
  • Serve as a preceptor for pharmacy students for hospital clinical rotations from affiliated pharmacy schools
  • Perform other related duties as required

Qualifications:

  • Doctor of Pharmacy Degree from an accredited College of Pharmacy
  • Completion of PGY1 Pharmacy Practice residency
  • Completion of PGY2 Specialty residency in Emergency Medicine
  • Skills in multi-disciplinary team involvement with direct patient care competency in Emergency Medicine drug therapy desired.
  • 2 years of clinical experience preferred.

Required License/Certifications:

  • New York State Pharmacists License, or eligibility to sit for the exam
  • Board Certification in Pharmacotherapy (BCPS) or Board Certification in Critical Care (BCCCP) preferred
  • Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) required
