The HSC College of Pharmacy (HSCCP) is located at the HSC campus in Fort Worth, TX and is joined by colleges/schools of Osteopathic Medicine, Public Health, Health Professions, the Graduate School of Biomedical Sciences and the UNTHSC/TCU School of Medicine. HSCCP graduated its first class and received accreditation from the Accreditation Council for Pharmacy Education (ACPE) in 2017.  

 

HSC Health is the academic medical center of the University of North Texas Health Science Center located on its’ campus in Fort Worth, TX. The center is comprised of 11 specialty clinics and a community pharmacy serving the people of Tarrant County and beyond. HSC Health seeks to push the boundaries of discovery through education and research as we keep patients at the center of our efforts. 

The integrated Ambulatory Care Pharmacy Services at HSC Health are responsible for comprehensive medication management and the provision of quality patient care to those referred/consulted by their provider. The direct patient care responsibilities of the Clinical Pharmacists are delegated through an established, broad Collaborative Drug Therapy Management (CDTM) protocol following Texas State Board of Pharmacy rules and regulations. 

 

The University of North Texas Health Science Center (HSC) at Fort Worth is inviting applications for a Clinical Faculty/Clinical Pharmacist to join our Ambulatory Care Pharmacy Services at HSC Health and the HSC College of Pharmacy (HSCCP) Department of Pharmacotherapy. This position will maintain a practice site and provide pharmaceutical care to patients at the HSC Health Center for Older Adults along with academic responsibilities related to the provision of experiential rotations for Doctor of Pharmacy candidates, PGY1 residents and PGY2 residents in collaboration with HSCCP. The clinical pharmacist also works collaboratively with our partners at Catalyst Health Rx to support the medication synchronization program. The goal of this partnership is to improve patient clinical outcomes and decrease total cost of care through both proper medication utilization and improved medication adherence.

Essential function(s) of this position:         
 

The University of North Texas Health Science Center at Fort Worth is inviting applications for a Clinical Faculty/Clinical Pharmacist to join our Ambulatory Care Pharmacy Services at HSC Health and the HSC College of Pharmacy (HSCCP) Department of Pharmacotherapy. This individual will serve as a faculty member who will enhance the UNTHSC vision and mission, while making meaningful contributions to the profession of pharmacy and the Tarrant County community.
 

Responsibilities of the position include but are not limited to:
 

* The clinical pharmacist will conduct clinical activities at HSC Health Center for Older Adults and is principally responsible for the provision of face-to-face and telephonic comprehensive medication management (CMM); 
*Works collaboratively as an essential member of an interprofessional team offering expertise in evidence-based, guideline-directed, and cost-effective pharmacotherapy to improve clinical outcomes for the older adult population.

*Conducts educational activities, promotes patient safety through adverse event management, and actively participates in clinic committees and quality initiatives.

*Actively participates as a preceptor for P1/P3 students during Introductory Pharmacy Practice Experiences (IPPEs) and P4 students during Advanced Pharmacy Practice Experiences (APPEs) from HSCCP.

*Provide face-to-face and/or telephonic comprehensive medication management (CMM) and coordinates the synchronized dispensing of medications through Catalyst, when applicable.

*Willingness to serve as a preceptor for collaborative HSCCP/HSC Health pharmacy residency programs.

 

Minimum Qualifications: 
-Applicants must have graduated with a PharmD from an ACPE- accredited pharmacy program with a pharmacy degree and be eligible to practice pharmacy in Texas. 
-Completed an ASHP-accredited PGY-1 post graduate residency training program or possess a minimum of 5 years of clinical patient related experience. 
-At the rank of Assistant Professor, the applicant must have zero to five (0-5) years of academic or clinical experience. 
-At the rank of Associate Professor, the applicant must have at least five (5) years or more of academic experience.
