We are seeking Medication Therapy Management (MTM) pharmacists with advanced training, Board Certifications or similar credentials to work in a progressive area where pharmacists provide MTM services directly to patients. The clinical pharmacist works in a multidisciplinary practice and collaborates with team case managers and provides one-on-one phone based consultations with patients. The Pharmacist will consult with the patient to identify, resolve and prevent medication related problems and assess each of the patient's medications for indication, effectiveness, safety and adherence issues and incorporates lab data in the assessment process. The pharmacist prepares a medication summary and personal care plan with recommendations for the patient to share with their medical providers and follows up with the patient to monitor the patient's drug therapy and to ensure positive outcomes.
Required Qualifications
Arizona pharmacist license required. Active and in good standing.
Passion for counseling patients

Prior MTM Experience is preferred.
Prior PBM experience
Education
Pharm D required along with registered Arizona Pharmacist, active and in good standing
