This position requires onsite travel periodically to all of our facilities.

Purpose:

Responsible to direct and coordinate a comprehensive clinical pharmacy practice and integrated program across pharmacy for Dartmouth-Hitchcock and its affiliate locations in both the ambulatory and hospital based areas. Provides leadership, strategic direction and goal planning for all medication formulary and clinical aspects of the medication use processes throughout the Dartmouth-Hitchcock Health (D-HH) system. Provides oversight of professional activities of clinical pharmacists and support staff in primary care and care transitions to provide standardization of the role and provide expertise to clinical integration throughout the D-HH system. Collaborates with Retail/Specialty Pharmacy and Oncology/Infusion Pharmacy system leaders to support practice advancement and practice standards in all D-HH ambulatory sites of care. Provides oversight of the residency training program(s) (ASHP accredited) in order to expand clinical and administrative rotation opportunities throughout the system. Works collaboratively with pharmacy leadership within the D-HH system and is a liaison with medical and nursing leadership for drug utilization and patient quality outcomes matters. Develops and implements system clinical goals and initiatives as well as uniformed clinical practices across the D-HH system. Works closely with pharmacy leadership in the design of financial stewardship programs and measurement of key pharmacy performance indicators.
  • Provides leadership and oversight of all pharmacy clinical programs and services within the system. Develops programs, goals and objectives consistent with those of the D-HH system and D-HH Pharmacy Services overall strategic plans and initiatives.
  • Coordinates with the site clinical pharmacy leaders in the ambulatory and hospital-based clinical programs and services to optimize patient care across the D-HH system.
  • Works with site clinical leaders to organize, plan and coordinate current clinical programs to meet organizational needs. Coordination of programs includes but is not limited to: medication use evaluations (MUEs), antibiotic stewardship, clinical utilization analysis and review to identify evidence based practice opportunities.
  • Develops and identifies metrics with the clinical leaders for monitoring programs in a performance tracking tool for the sites and applicable programs.
  • Serves as the clinical pharmacy leader at the System Pharmacy and Therapeutics Committee.
  • Supports the Medication Management related activities and works with Pharmacy leadership to help ensure Joint Commission chapter compliance.
  • Supports the development and evaluation of system policies and procedures, and assures all facilities in prevue are similarly engaged. Supports development, when necessary of Standard Operating Procedures enabling facility compliance with system standards. Ensures compliance with state and federal regulatory agency standards.
  • Participates in Pharmacy safety focused initiatives with the pharmacy leadership team in the areas of medication utilization and evidenced based practice.
  • Works with system leaders to identify and maximize ACO opportunities related to pharmaceutical spend and optimizing patient care activities.
  • Assists in the strategic planning of the annual programs, operating and capital budgets as well as the long term planning for system pharmacy initiatives.
  • Works with D-HH clinical leaders in clinical staff education and development programs.
  • Reviews and presents the clinical program and financial overview on a semi-annual basis.
  • Manages staff and leadership resources by overseeing the recruiting, selecting, hiring and orienting of qualified individuals. Additionally, supports a comprehensive approach to professional development of staff. Evaluates staff performance in an accurate, consistent, objective and timely manner. Develops core position hiring strategies.
  • Creates a cohesive pharmacy team, both at local and system wide levels, committed to providing operational, clinical and service quality to all customers. Establishes a succession process/plan for development of leadership positions and increased staff engagement that improves communications, builds teamwork and optimizes job functions to maximize efficiency.
  • Works collaboratively with all pharmacy leaders and key medical, nursing and administrative leads throughout the D-HH system to ensure strategies, goals and objectives and processes are aligned.

  • Performs other duties as required or assigned.

    Minimum Qualifications:
    • Pharm D.
    • MS in Healthcare Leadership/Healthcare Management, MBA or other advanced degree preferred.
    • Board Certified Pharmacy Practitioner (BCPS) preferred
    • Minimum of 10 years clinical pharmacy leadership (5 must be in hospital pharmacy) or the equivalent required.
    • Demonstrated ability to work independently with minimum supervision, and possess the ability to set priorities and make critical decision.
    • Managerial effectiveness, problem solving, and change management abilities desired.
    • Effective oral and written communication skills, as well as demonstrated interpersonal and team skills essential.
    • Travel up to 50% of time. Most travel is within system and regionally.

    Required Licensure/Certification Skills:
    • Graduate of an ACPE accredited school of Pharmacy
    • Current unrestricted NH licensure (or licensed within 90 days of hire)
    • Board Certified Pharmacy Practitioner (BCPS) current certification if BCPS certified
