Responsibilities: The clinical pharmacist is responsible for a high level of direct patient care and assures optimal drug therapy primarily, but not exclusively, for patients with cardiovascular disorders. The pharmacist is responsible for clinical pharmacy services including but not limited to: participating in medical rounds, collaborate with other healthcare professionals, conducting medication reconciliation, reviewing medication orders for compliance with hospital policies, educating staff, and optimizing medication therapy to achieve the best outcomes possible for patients. Additionally, the pharmacist will oversee the medication distribution system for their area of responsibility. The clinical pharmacist will also be an active contributor to the cardiology area by working to develop policies, procedures, guidelines and protocols.

Participation in the training and education of pharmacy technicians, residents, students, interns is expected. Assures the implementation of processes to assure the quality of Pharmacy services. Interacts with co-workers, visitors, and other staff consistent with the core values of the Hospital.

Qualifications

  • Doctorate of Pharmacy Degree Graduate from an accredited college of Pharmacy
  • PA Pharmacy license
  • Successful completion of PGY-2 residency in Cardiology (preferred) or 2 years of relevant experience
