Responsibilities:
The primary purpose of the Clinical Pharmacist Specialist (ED) position is to optimize the use of patient medications for all ages and populations through participation in direct patient care rounds, medication order review, medication therapy monitoring, participation in procedures that utilize high risk medications, resuscitation, medication procurement and preparation, provision of medication information, and documentation of associated interventions. Works collaboratively with healthcare team to accomplish departmental goals and improve patient outcomes.
Qualifications:
Candidate must possess a Pharm D., or Bachelors Degree from an accredited school of Pharmacy with Board Certification in a specialized area. Pharmacy Practice Residency or equivalent is preferred. A minimum of one (1) year of hospital experience with enhanced clinical responsibilities required. Must obtain ACLS and BLS certification within six (6) months of hire. PALS preferred. Must be available to rotate through some weekends.
