Ambulatory Care Pharmacist - Cardiology and Anticoagulation
The Department of Pharmacy and Mayo Clinic Northwest Wisconsin (NW WI) Division of Community Cardiology and NW WI Primary Care Anticoagulation Clinic are seeking an Ambulatory Care Clinical Medication Management Services (MMS) Pharmacist to join our team. This is a full-time position working 8 a.m. to 5 p.m. Monday through Friday, based out of Eau Claire, WI. This role will be divided 0.7 in Cardiology MMS & 0.3 in Anticoagulation. The individual in this role will be responsible for MMS in Cardiovascular peripheral arterial disease and expanding to other complex medication regimens within cardiology. The anticoagulation coverage will meet critical needs for vacation for both NW WI & Southwest Wisconsin. As part of this role, the pharmacist will be responsible for patient and provider questions, team meetings, quality improvement, and practice management and expansion in addition to patient care activities. Involvement in committees and quality improvement is expected, and research and precepting are also possible within this position. Working under a collaborative practice agreement is part of this role.
This Cardiology MMS & Anticoagulation Pharmacist will work with a variety of team members including: cardiovascular physicians, social workers, psychiatrists, case managers, and nurses to evaluate, plan, and monitor therapy.
Qualified applicants must be licensed to practice Pharmacy in Wisconsin, a graduate of an accredited College of Pharmacy, and have a Doctor of Pharmacy degree. Completion of an accredited pharmacy residency in cardiology or experience working in a cardiology ambulatory care clinical practice is preferred. BCCP, BCACP or BCPS as well as completion of an MMS certificate program are preferred.
