POSITION SUMMARY: Performs Clinical Pharmacy activities at UT HEALTH EAST TEXAS-Tyler under the direction of the Clinical Manager of Pharmacy. Provides Clinical Pharmaceutical Care to patients in order to assure appropriate use of drugs and IV solutions while minimizing the likelihood of adverse drug events. DUTIES, ACCOUNTABILITIES AND RESPONSIBILITIES include the following: Manages Pharmaceutical Care through direct, concurrent and retrospective review of patient medical records to provide optimal medication therapy Implement programs for monitoring and improving drug and IV use at UT Health as directed by the Clinical Manager of the Pharmacy Documents activities in Pharmaceutical care to show improved patient outcome and/ or improved cost-effectiveness of medication use Facilitates and/ or serves on committees as requested to represent the Pharmacy in matters where Clinical Pharmacy input is needed Assist the Clinical Manager in developing and managing the education programs provided by the Clinical Pharmacy Department Facilitates implementing and monitoring clinical protocols and guidelines, and enhancing the delivery of evidence-based medicine Provides good customer service by utilizing good communication skills and technical knowledge to ensure the objectives of UT Health and the department are maintained Works quickly and accurately; able to prioritize work demands SUPERVISION: This position assists the Clinical Manager of Pharmacy in advising the Staff Pharmacists in Clinical Activities. QUALIFICATIONS / REQUIREMENTS: Education: PharmD with PGY1 residency preferred RPh with BS in Pharmacy considered with 3-5 years Clinical Pharmacy experience Experience: As described above under education Certification and Licensure: Licensure from the Texas State Board of Pharmacy and evidence of biennial renewal PHYSICAL REQUIREMENTS: Ability to sit or stand for long periods of time while completing work on a computer Frequent walking Ability to use verbal and non-verbal sources of information to effectively communicate with other health care providers May require minimal lifting effort (up to 25 pounds) on a rare basis Must perform activities requiring sustained concentration ENVIRONMENTAL CONDITIONS: May involve some contact with potentially hazardous or harmful chemicals in providing administrative or support services Frequent contact with patients Environmental Conditions: Exposure to infectious disease on a constant basis; Exposure to ionizing radiation on a frequent basis; Exposure to unpleasant working conditions or undesirable elements, some of which are potentially hazardous or harmful, and which may require exercise of well defined precautions for protection from injury or illness; Potential for cuts, bruises, and muscle strains.
